My own unsolicited opinion If you're not a doctor and use the word 'consultant' in your job title in a hospital setting then your title should also have your professional background in it For eg Consultant Nurse Consultant Paramedic, Consultant Physio etc etc

Can't be understated the moral injury being suffered by paramedics and all HCPs in amb services and wider NHS at the moment Going to work in a broken system knowing that you'll be frustrated due to workload and not able to provide the care you want is horrendous

I genuinely believe that there is a lack of understanding by a significant amount of hospital based clinicians of the level of risk held out of hospital by GPs, community teams, Paramedics and ambulance services amongst others.

I realise as a paramedic who no longer works frontline in an ambulance I'm probably not the best person to talk about this, but I really think our profession needs a reckoning with making frontline ambulance work more appealing for the longer term, as it's clearly not at present

It seems that physicians associates (2 years "trained in the medical model") are seen as equal to advanced nurse and clinicial practitioners (i.e heavily trained specialists) and GPs (actual consultant doctors with 5 years medical school + 5 years minimum training.) Its a joke.

I noticed on another ward they have a trainee ACP, and when the cons isn’t there 3x a week, the trainee ACP sees half of the consultants patients for a WR (JD sees the other half). We would not let a third year med student do this, so why a trainee ACP? Peculiar.

@HelenRSalisbury Dr Salisbury we can't underestimate both the unseen harm in the community through lack of ambulances due to this and the significant l moral injury for paramedics who can't provide level of care they want to and are left idle for hrs on end knowing there are pts waiting elsewhere

In the same way I argue ANP/ACP ≠ GP as no1 can be equal of a GP without being a Dr and going through medical school. Equivalences are awful when trying to benchmark practice But, without a doubt in my own mind, PAs are not the equivalent of ANP/ACP practice

Yes not every referral/conveyance is perfect But the sheer level of risk taken on and the amount of work diverted away from EDs is significant For every "poor" referral/conveyance there are stacks of patients being dealt with and treated appropriately in the community

A trainee ACP will be a regulated individual and have significant post grad experience (usually a minimum of 5 but very unusual to get a post 5 years post grad) If you think a 3rd year medical student is equal to a trainee ACP then I really don't know what to say 🫠🫠

This is awful for all concerned. Ambulance crew who had just 10hrs off after an infant cardiac arrest, crashed into a vintage car just before end of their next 12-hr night shift whilst driving through the same village as the call to the deceased baby. #HumanFactors #ThoseWeCarry

Imagine I told another profession where they should work? As doctors rightly say they operate in a competitive international job market where their skills are highly sought after Paramedic skills are sought after in the UK. They will work where their skills are sought

@ExplosiveEnema I'll only comment about myself I've 10 yrs frontline amb experience. 4 1/2 yrs in GP after MSc. I'm a prescriber I see 20 pts per day in 20 min appts. There are a good few things I can't see as don't have the training/skills. I won't work without senior GP from practice there
