Barista: You just finish an 8? 12?
Me: 28
Barista: *lowers mask to show jaw on floor* 28? Do you get paid extra for that?!
Me: Nope, I’m salaried...I get paid about $16/hr.
Barista: But your badge says your a doctor?
Me: Mmhm.

Just to play devils advocate….aviation industry also doesn’t schedule their employees to work extended shifts. Medicine does. Hospitals profit enormously from this long held notion that surgeons (and physicians broadly) should just work 30hrs straight, instead of hiring more Drs

@sargsyanz
I used to phrase that question that way, and then I realized that wording was putting the onus on the patient instead of the care team. Now I ask, “What have your doctors told you about what’s going on with your body and why you’re in the hospital?”
@DrTobiasFunky
This is 1000% false. If I ask if a medical student “wants” to do something—see a pt, do an exam, sew a lac, help w wound care—100% a test. If I *tell* then to go home, it’s because there’s no more hands-on learning to do and I chart faster without them looking over my shoulder.

1) These tests cost hundreds of dollars; incentivizing multiple attempts rewards privileged students and disadvantages others.

Even crazier idea: standardize parental leave so there aren’t wildly different parental leave policies out there and so that ALL of them are adequate/meet the recommendations we give patients!
@S__Baksh
Program directors seem to think when a union exists somehow their hands are suddenly tied—not the case. They can keep advocating for residents, in fact it helps to have attendings in support of their residents. Blocking unionization is not supportive. This is gaslighting.
@LVSelbs
Having a system that puts doctors in a position of having to choose between “doing the right thing” and flying home for the holidays is what leads to burnout. A functional system would ensure there’s another doctor there to do the right thing.
@G_Price25
@chrissyfreeMD
Yeah…
“Trauma fellow” = NP fresh out of school doing a yearlong training program in trauma vs
“Trauma fellow” = MD who completed general surgery residency (5-7yrs), surgical critical care fellowship (1 yr) now in final year before attendinghood.
Not equal.
@olsonplanner
Bigger issue is that intern orientation often isn’t paid, and then paychecks are usually monthly, so your last student loan disbursement is ~March….and then first paycheck August 1.
@ESilvermanMD
More residency programs. That’s the rate-limiting step. We have unmatched medical students every year and no shortage of IMGs who want to come here to work.
@EbunoluwaFalade
I just keep breaking it down by syllable and ask them to repeat each one (for my last name). They can stand feeling uncomfortable/like a five year old for a minute. And then I say “See! Not that hard!”
@ChrisHornMD
This happens too frequently. A woman passed out on a flight I was on recently. Luckily multiple medical professionals were on board- an OBGyn, NP and ICU nurse. Yet flight attendants kept addressing the OB's husband? Finally he exclaims, "Don't look at me, I'm an accountant!"
@Sharlita62

Fun fact CA has a state law that our UC hospitals can’t give free parking to staff! The fact that we don’t fund parking at public hospitals through taxes as a public good is mind boggling to me.
@mhumphriesmd
@farkomd
@Skepticscalpel
That’s a false comparison. Medical students are physicians in training, using a scalpel properly is a relevant skill. Also, OR consent forms state that trainees will participate/assist in the case. As a patient I’d be furious if a random man was allowed to cut me just for kicks.
@DocWithBowtie
The Medicare 3-day rule serves as a blunt tool to ration nursing care/facility beds and would be wholly unnecessary if we had adequate long-term care for the aging in the US.

I love our APPs too! They are a crucial part of the team. But I don’t think we in surgery can fully appreciate what is happening in primary care and EDs where MDs/DOs are being fully supplanted because hospital systems are putting profits ahead of patients.
@KimiChernoby
Because “residency” after a PA or NP program is optional, the salaries for this speciality training has to be competitive with other NP/PA jobs. To me the huge disparity in pay just signals how much medical residents are undervalued, not that PAs/NPs are overvalued.
This is one the most infuriating takes I’ve seen in a while.
Societal attitudes towards women’s health issues and historical exclusion of women from medicine/research etc are WHY these issues are underdiscussed, under-recognized and underfunded! So…
Ladies: your pelvic floor is one of your most precious commodities for healthy aging.

FYI…most people graduating med school are minimum age 26, which is older than the IRS limit for claiming a dependent.
Also, allowing parents to claim you as a dependent during any year of medical school can disqualify you from grant-based aid, if your school offers this.
@ryan_barnicle
I can help demystify:
- MRN first: I’m sitting at a computer
- Story first: I’m busy doing something, and trying to triage when to fit in consult
- Room number: It’s a simple/visual consult (e.g wound) and can probably be seen before I log into a computer to see pt location.

1) Make sure medical student applicants for the 2021 cycle knew this. They deserve to know how much the hospital administration values their lives.
2) Organize. A demonstration of residents handing over their pagers as attendings are getting shots might create a vivid picture.

Locking in disability insurance immediately is good advice for ALL surgical trainees (regardless of sex/gender/family plans).
But let’s talk about how it’s more expensive for women. Regardless of age. Apparently having a uterus is a pre-existing condition.

A riddle for you:
A man is driving his 6-year-old son.
Sadly, a car accident claims the father's life.
The son is brought to a trauma center, where a surgeon is waiting in the trauma bay
The surgeon says: "I can't operate on this patient. He's my son."
Explain.
This absolutely appalling.

should be embarrassed to claim these are adequate working conditions. This is no way to support women in healthcare. Do better.

As a surgeon I can tell you each year there are more qualified applicants than there are spots. So the question becomes do we want a society in which people in all privileged positions look the same? Have the same cultural perspective? Surgeons do more than cut.
@emergcasey
We should be supporting professional development of all trainees. ACGME training programs have to demonstrate sufficient procedural training opportunities as part of their accreditation. Shouldn’t NP/PA training programs have to do the same?
I didn’t think today would be the day I ran to the ED to see an unstable patient and have a team of people look at me and say “Oh good! Surgery is here!” Guess I managed to conceal my own panic?

I agree using the term “residents” for anyone other than physician trainees is confusing. Also ironic the term came from when they literally lived in the hospital, which 4 10-hr shifts a week definitely is not.
