Clinical trials are more important than ever today amidst this pandemic. Here are some thoughts we published in
@NatRevCardiol
on how
#COVID19
may help catalyze change to improve the clinical trials process!
@DLBHATTMD
@BrighamWomens
Last night in the ICU of a
#NYC
hospital, I cared for 20 patients who were all on breathing machines due to
#COVID19
. Some REALLY young (20s), without comorbidities. Everyone is extremely sick. But sicker patients keep flooding in…
But I have never been prouder to be a physician in
#NewYorkCity
. My incredible colleagues - everyone - is rising to the occasion. We are doing our best. We will get through this together.
It is heartbreaking to have end-of-life discussions w/ family members. No part of me was prepared to convey that a family member can’t say goodbye. That their love one would have to be alone during their final moments. I cannot imagine how they feel...how painful it is...
Some of our sickest patients with
#COVID19
really need dialysis, but we don’t have enough machines. We are actively rationing care - something I thought I would never see in our healthcare system...
We are REALLY feeling the strain. We don’t have enough ICU beds. Many patients are being intubated as soon as they show up to the Emergency Room. We are doing our best to take care of them on regular floor beds and in the ER itself. But it is HARD...
What a WHIRLWIND week it’s been in
#NYC
! More and more patients with
#COVID19
coming in. Our emergency room has become, in part, a fully functioning ICU as inpatient ICU beds are struggling to keep up with the volume...
Amidst all the chaos working on the front lines for
#COVID2019
at
#NYPColumbia
, NYC clapped today for all healthcare and essential workers! So much work to do up ahead but thank you
@NYC
❤️
Published today in
@CircAHA
, we show that in FOURIER-OLE, lower LDL-C levels, down to <20 mg/dL, were associated with lower adverse cardiovascular events without significant safety concerns in the long term.
More reassurance to target lower!
@TIMIStudyGroup
The camaraderie has been absolutely INCREDIBLE. Nurses and doctors across all specialties (Derm, ENT, Urology, and more!) are working tirelessly together, doing everything they can to tackle this pandemic.
Officially graduated from Cardiology Fellowship
@Brigham
today!
@BrighamFellows
@harvardmed
Could not have asked for a better or more fun loving crew of cofellows to be in the trenches with🫀
Cheers to all!
So much interest in Lp(a)!!
Had an awesome time presenting our study investigating Lp(a) variability among patients from the OCEAN(a)-DOSE trial
CVi 10%
Max abs change of 135 nmol/L
Guidelines recommend testing Lp(a) once in a pt’s lifetime.
Time to rethink?
#AHA2023
Many
#COVID19
patients have been intubated for longer than I would have expected. But there have also been uplifting moments. Some of the patients I have cared for have been extubated and are recovering.
There’s still LOTS of work up ahead, but hopeful that teamwork both in the hospital and outside of the hospital (
#socialdistancing
#stayathome
) will help us further flatten the curve during this unprecedented time.
Absolutely thrilled to have had the opportunity to present a late breaking clinical trial

Does PCI, without anti-anginal medications, improve anginal symptoms over 12 weeks of follow up among those with stable angina?
Short answer: Yes!
Thrilled to have presented our analysis investigating aortic stenosis events in the long term among patients treated with evolocumab vs placebo in the FOURIER and FOURIER-OLE trials
#TCT2023
Higher Lpa and LDL-C lvls were associated with a higher risk of aortic stenosis events!
Excited to share our new study now in
@JAMACardio
, highlighting outcomes in pts with left main disease and acute coronary syndrome after PCI vs CABG. Risk of 5-yr death similar, but tradeoffs for nonfatal events.
Huge thanks to
@BrianBergmark
and Dr. Sabatine!
@TIMIStudyGroup
In pts with LM disease, those with ACS had higher rates of early CV death and MI. Risk of 5-yr all-cause death was similar with PCI vs. CABG in those with and without ACS; tradeoffs for nonfatal events with PCI vs. CABG were consistent regardless of ACS.
In
@JACCJournals
today, Dr. Eugene Braunwald shares his 6-decade view of aortic stenosis diagnostics and therapeutics.
Potential key mechanistic contributors of AS include Lp(a). With therapies for Lp(a) on the horizon, this is an exciting time!
BREAKING GOOD NEWS ALERT:
New Zealand has announced it no longer has any patients in hospital being treated for coronavirus.
It comes on the fifth straight day where no new cases have been reported.
This is what winning looks like.

In our hospital, several
#COVID19
units are being converted back to regular floors to allow care for patients with a variety of different health concerns.
After being on solely
#COVID19
units for the last few months, this transition definitely feels surreal!
47 New York counties can now resume elective surgeries.
Ambulatory Surgical Centers in those counties can also now resume elective surgeries.

I love
#NYC
. But reopening the city now could lead to a major setback and even more heartbreak.
We do NOT want to reverse the progress we have made together.

Recruitment practices for randomized clinical trials vary across high and low recruitment centers, potentially biasing study results.
In this editorial of a thoughtful article by Ndrepepa et al.,
@DLBHATTMD
and I suggest ways to improve this process.
In the first LBCT session
@CRT_meeting
2023,
@DLBHATTMD
presents key results from SIMPLICITY-HTN 3, suggesting an important role for renal denervation in black patients
In this sub-analysis of the EXCEL Trial, we found comparable rates of MACE in selected patients undergoing outpatient versus inpatient unprotected left main coronary PCI. Now published in The American Journal of Cardiology.

Today, in
@EHJ
,
@DLBHATTMD
, Dr. Boden and I share our perspective on considering greater use of icosapent ethyl for patients with hypertriglyceridemia and atherosclerosis.
We must weigh the pros and cons of each patient’s unique clinical scenario!
Very excited about this! I've been incredibly fortunate to work with truly *exceptional* mentors, colleagues, collaborators, trainees, and students, who all made this possible. Thankful for my family and friends.
"The Team, The Team, The Team"
A wonderful experience at the Green dinner yesterday - one of the first official fellow and faculty celebrations since the pandemic!
@BrighamFellows
@Bweber04
Today, there is no effective pharmacotherapy for aortic stenosis. In this
@CircAHA
study,
@AeronSmallMD
and colleagues found several novel genomic regions associated with the condition.
Calcific aortic stenosis is common and has no effective medical therapy. We performed a multi-ancestry GWAS of 14,451 CAS cases in the Million Veteran Program, identifying 6 novel genomic regions and doubling the known GWS loci in CAS.

piece notes the potential for coagulopathy (formation of clots in vessels of various body organs) in patients with
#COVID19
.
Something I too have observed while taking care of patients.
Recent increases in
#COVID19
cases all over the country are disappointing and stress the importance of social distancing and wearing masks as we enter the fall and winter seasons.
We cannot let our guard down.
For those that have followed the controversy regarding
#FOURIER
brought on by the authors of the recent BMJ Open article, please take a moment to read this crucial explanation of facts.
“In FOURIER, event adjudication followed a rigorous, pre-specified, blinded process…”
Now in the late-breaking clinical trials session at
#ACC2023
, Nissen and colleagues reveal significant reductions in MI (23%) and coronary revascularization (19%) with bempedoic acid.

New clusters of
#COVID19
in Asia stress the importance of testing and tracking prior to reopening major cities.
Let’s learn from this
#NYC
. And prevent a resurgence.
1 million New Yorkers have now been tested for COVID-19.
NY is doing more diagnostic tests per capita than any other state — and any other nation.
We are working tirelessly to increase testing capacity even more.
A momentous moment for women in America.
“While I may be the first woman in this office, I will not be the last—because every little girl watching tonight sees that this is a country of possibilities.”
While I may be the first woman in this office, I will not be the last—because every little girl watching tonight sees that this is a country of possibilities.
The hardest part of this has been seeing the heartbreak and emotional pain that many patients' families experience. Sharing that their loved one is getting sicker or not going to make it after days (or in some cases, weeks) on a ventilator... it is utterly devastating.
