I did find this shocking - in comparison our nurse hysteroscopists do a postgraduate qualification requiring >200 supervised cases before independent practice. They are highly qualified. I find the training here a little dubious...

I think we need to emphasise that it is not just impacting senior clinicians and forcing them to retire early. It is impacting relatively new consultants,like me, forcing us to limit and reduce activities which is ridiculous in times of huge backlog!

Really important to be vigilant of symptoms particularly if on HRT
Perhaps we could focus on what matters to the majority of patients on these womens wards the most (mostly gynae) - the horrendous waiting lists, the chronic lack of investment in womens health, the healthcare inequalities in womens health etc etc

The NHS waiting list doesn’t need more surgeons it needs more capacity.
There are surgeons all over the UK with one shared list per week. Maybe even one shared list per fortnight.
They can operate much more than that but there aren’t the theatres, wards, ICU and nursing.
Sharing this from 2013 - MRCOG done! Was such a relief to pass and got a gold medal for my efforts 🏅 Well done to everyone admitted to the college today!! Such a special moment - take the time to enjoy it!!!

so a few of my thoughts...Women's health has always taken a back seat, never a top priority for trusts even pre-pandemic. If we compare the funding differences between for example urology and gynaecology - its is huge. Trusts are paid a-lot more for urological procedures
Patients are not allowed access to the best resources as it is just "benign" gynae
Money is not allocated as it is just "benign" gynae
Why is this?

Delivery following myomectomy - the evidence (+my thoughts)
1. Incidence of uterine rupture -
Open myomectomy - 0.4 - 1.7%
Lap myomectomy - 0.49 - 1.2%
Comparable to prev LSCS 0.5-1%
When ruptures do happen with prev myomectomy - they tend to be PRE-LABOUR (70% >36 weeks)

1/2 Saw a patient yesterday and they asked me "am i allowed a hysterectomy?"
This made me both sad and angry that a patients feels like they need to ask permission for a treatment that is very likely to cure their symptoms.
Their body/their choice/ informed consent
Can’t believe that today a gynaecologist told me that a hysterectomy wouldn’t be considered as an option for my debilitating period pain on the off chance that I divorce my wife, my sexual orientation changes, I meet a man and decide I want children 🙃🙃🙃
ICG again - following on from last post. Case of frozen pelvis, prev hysterectomy, ovaries imbedded with dense bowel adhesions. Used ICG - initially unable to see due to significant fibrosis, but once the pelvic side walls were opened, I did find it a helpful adjunct to help...

Endo symptoms might improve after the menopause, however a large number of women still experience symptoms after natural and surgical menopause.
Symptoms may worsen with the use of HRT particularly oestrogen.
