@wesstreeting Dear Mr Streething, I fucked off to Canada after CCT because the NHS you so proudly run imposed a hiring freeze and I couldn’t get a job. Now I’m back doing a bit of locum work and earning serious coin during the strikes. Thanks for pissing off another generation of doctors.

This comment on my HSJ story about the resident doctors' pay offer is likely to raise some hackles. I do think things have changed almost unrecognisably for resident doctors compared to those in consultant posts now - and probably not for the best.

Wes @wesstreeting The same-day emergency care (SDEC) unit in my hospital is staffed by: 🔻2 ACPs (god knows what base profession) 🔻2 ANPs (nurses) 🔻4 physician assistants 🔻1 clinical pharmacist This means 9 doctor posts are permanently occupied by people who are allowed

@wesstreeting For context, other NHS workers get 50% extra per hour for enforced overtime above 37.5 hours. Doctors get none of that and when this unfair discrepancy was suggested, you shot it down.

@DrNeenaJha A first year Trainee ACP ordered X-rays on me, reviewed them and discharged me. I was pushed into a waiting area on a wheelchair shaking violently. I attended a different hospital 10 days later ..... L1 compression fracture, 3 pelvic fractures and 2 posterior rib fractures.

I think there are very real issues here, and I think we devalue and insult our doctor trainees by using the language of equivalence carelessly. They are not in a position to and do not feel safe commenting.

ACP training is focussed on managing the everyday ED attendance. They are taught pattern recognition and more importantly when the pattern doesn’t fit and when to ask for help. They are not, and never will be, equivalent to senior doctors.

I'm shocked BMA officials urged GPs to divert urgent patients to A&E to 'overwhelm' services, as protest. It’s unacceptable - patients must never be used as leverage. Both GPs & patients want & deserve a health service fit for current & future generations.

Can we please stop using “but there aren’t enough doctors” as an excuse for filling medical gaps with people with no medical training? 1. If there aren’t enough doctors the only safe response is to recruit/retain doctors. 2. Most importantly: there ARE more than enough doctors.

It's the 27th September. About 8 months since this case raised to @CareQualityComm "It's under investigation" is- I am afraid- not enough So- we ask further. A Physician Assistant did a neurosurgical procedure on an unconscious patient- supervised by a trainee-as per notes
