Not a {Consultant Practitioner in EM- BSc, MSc, & PhD in Advanced Clinical Practice}.

That’s the cocktail: individuals from different professions completing a part time MSc in Advanced Practice & then all holding the title of ACP in EM or in a speciality, doing the same job once carried out by Drs - seeing serious & undifferentiated cases. This is not safe at all.

Why do people in positions of power not speak up-when they see something perceived to be wrong? In my experience with @RCPhysicians @gmcuk @NHSEngland @NHSGIRFT @PHU_NHS - there are about 5 reasons. Via @bmj_latest

The leadership journey for me started pretty early in my career—as a trainee, in fact—before I moved on to a clinical director role within a year of my appointment. When I worked in local leadership...

We are about 12 months away from Royal Colleges having to make a statement around ACPs and “doctor substitution” Unless job opportunities increase for doctors? It’s coming. Prepare for it. Be ahead of the curve before you lose control over this one too Look beyond your

Wes isn’t reducing waiting lists There aren’t 2500 more GPs Who is delivering appointments in the NHS, it’s the staff, not Wes or Labour & in spite of them not because Wes’s own constituency has some of the worse GP access He needs to go!

Doctors supposedly pay the GMC to ensure that only Doctors practise medicine But we are seeing more and more that Trusts are employing Noctors to work on Medical Rotas as pseudo-Doctors So it really begs the question… What are Doctors paying the GMC for?
@VincentVanGrump @LostDazes @ExplosiveEnema2 Why doesn’t this matter? And if it doesn’t matter why are Drs paying the GMC?

ACPs don't replace Doctors 2 years training ⬇️ Doctors Crit care rota 🙂 There is a place for Advanced Practice. The Doctors Critical care rota is not that place.

ACPs don't replace Doctors "however their clinical decision making and clinical level of patient management will reflect that of a middle grade doctor, having the clinical ability to work on a medical rota if required" 🙂

⚠️I keep reading how ACPs and PAs supposedly “help” with “doctor shortages”. ⚠️However nowhere is mentioned that these “shortages” were artificially created by withholding funds for actually employing all of the Doctors out there. Does our Public actually realise the difference?

No one can work as a DOCTOR in GP without min 5yr at Medical School, 2yr general hospital roles & 3yr GP training Then they can consult undifferentiated patients with undiagnosed problems Yet @NHSE_WTE decided this was OK for a PA This is a patient safety issue for the UK

She is not leaving. Emily was our trainee -now an excellent &experienced GP. She is being *made redundant* at a time when patients are desperately to be seen & treated by such amazing GPs. @wesstreeting’s GP replacement agenda really has been the final nail in the coffin. ⚰️.

In 1996 we had 27,550 FTE GPs in England So in 29 years we have gained 151 FTE GPs 151 extra GPs to look after >10million extra patients Wow - this is a huge accomplishment by NHSE workforce planning
So, we now have fewer fully qualified, permanent GPs-down from 28,680 in 2016 to 27,701 in 2025. What an impressive increase. Meanwhile, the number of non-doctors practising as GPs in primary care keeps rising, compromising safety but no one dares to challenge the big players.

The NHS workforce is growing, but demand for care is also rising and health needs are increasingly complex. While headlines often focus on overall staff shortages, the reality is more nuanced. Find out more.

Patients desperate to see GPs. Unemployed GPs. Alternative professionals employed instead of GPs. @wesstreeting our GPs deserve more and our patients deserve better.

This is misleading from @TheKingsFund in terms of their graphic regarding GPs Including GPs in training in the overall number distorts the real picture of GP provision Many GPs are failing to get jobs after training demonstrated by the numbers of FTE fully qualified GPs

It is quite extraordinary how massive a workforce crisis is about to explode on us. And all national organisations are practicing who can be the best ostrich You have to head the crisis off early Not react when it comes. Ah well. What do I know 😊

Good question
@medicalmodelbri Could trainees, Boycott the colleges? Appeal to the entity that grants their power? (Royal assent 🤷♂️?) Petition/cajole voting members to have a more doctor friendly attitude? Highlight any conflict of interest in voting members? Create a doctor friendly voting block?

Guess what happens when people without regulation and training qualifications work in healthcare It’s a shame journalists like @ShaunLintern fail to apply the same journalistic rigour when it comes to doctors But? They all will as the safety incidents start stacking up.
Kingdon's report will call for audiologists to be professionally registered and only allowed to work in the NHS if they meet certain standards. Currently anyone can call themselves an audiologist with many working in the NHS not registered with any organisation.
