I am very excited today to announce my next adventure,
@MyMarleyMedical
, a new virtual-first primary care clinic for people with common chronic diseases. Excited to partner with the awesome
@julesyoo
and
@kbakes
on this journey!

After 6 incredible years
@PropellerHealth
, today it is with very mixed emotions that I am saying goodbye. Propeller has been, by far, the best career experience of my life. I am proud of the team and all that we accomplished. Below is a thread with some great memories. (1/23)
Very excited for our big announcement! Together
@PropellerHealth
and
@ResMed
can dramatically improve the treatment of chronic respiratory disease. And i could not have taken this journey with a better team!
Pear Therapeutics announced that they are filing for bankruptcy, which made me sad. Although my thinking has changed over time on DTx, I am still a fanboy at heart, feel pretty lucky to have seen this whole arc and feel like reminiscing.
I think I’m a contrarian on the

@Ascensionorg
news. Google is a clear Business Associate in this arrangement, and unless they violate HIPAA in the use of Ascension’s data, I think they can add a lot of value to (very siloed) data assets. And benefit patients and docs.
@Avik
I usually like your opinions, as they differ from mine. This is beneath you. This is an obvious example of poor planning and poor execution. Let’s go back to oil lamps if we are good with 18th century processes.

Once again, the rumor of the death of digital health+pharma partnerships is greatly exaggerated.
I think this is the first time we’ve spoken publicly of our work with AstraZeneca ✅
We're excited to share that Propeller has received FDA clearance to connect patients using the Symbicort Inhaler to our platform, a huge milestone in bringing digital health to people with asthma and COPD.
In healthcare the success of innovative companies is mostly determined by go-to-market strategy. Building a great product/solution is necessary but not sufficient, unfortunately.
“Propeller Health’s genius is perhaps not in developing smart sensor technologies but in defining and measuring patient outcomes such as medication adherence, increase in symptom-free days and a reduction in emergency room visits.”
!!!
As for me, I am taking a bit of time off to re-charge the batteries and spend time with family (including our newest member below), and then will figure out what is next. But
#DigitalHealth
and entrepreneurship are addictive, so I will be back in the arena soon enough. (23/23)
Starting a NewCo and am raising $5M at $50M. All I can say is that we are going to assemble the best technologists and scientists in the world and we will use the latest in AI and computation to examine the health records databases of our nation to solve chronic disease. DM me…
Patients with asthma and COPD are at increased risk during COVID-19. At
@PropellerHealth
we wanted to see if patients are following guidance and diligently taking their daily controller medications. They are! We see a dramatic increase in adherence of 14.5% in last weeks. (1/3)
Really enjoyed my time at
#ViVE2023
. Bonding with other founders is different in down markets. It’s better and more real. I am leaving Nashville feeling filled up and optimistic.
This definitely makes me sad. Proteus was one of the first companies that got me really excited for the future. I’ve been putting Proteus in decks for over a decade! Regardless of outcome Proteus is a super important company. 😢
Important post from
@seanduffy

5 years from now ‘Digital Care’ companies will be as prevalent (and funded) as DTx companies are today. In fact, I bet many DTx companies will pivot into being ‘Digital Care’ companies.
Anyone else planning to analyze the Oscar S-1, the bar is very high.
"The annual Oscar tradition of lighting piles of money on fire each year as a tribute to their membership growth deity is readily apparent in the financials." -

I know I am biased, but this is a BFD.
Docs in the EU will be able to Rx this newly approved NVS asthma drug alone or Rx the drug + Propeller Co-pack (drug+sensor+Propeller app).
Today, Propeller Health announced an industry first for digital health: our sensor will be co-packaged and available to co-prescribe with a new maintenance medication for asthma in Europe.
We have learned a lot in respiratory over the past 8 years. It’s time to spread our wings and apply those learnings to other diseases. Excited for this next chapter.
Now makes sense why I could never understand anyone trying to explain what Transcarent was going to do. It’s because it’s going to do everything. Made my brain short circuit. Makes sense now.

As a longtime fanboy of Oak Street Health, I was excited to see
@CVSHealth
(finally) announce they were acquiring
@OakStreetHealth
for $10.6B, a valuation of $60k per at-risk member. And I have thoughts!
Like many others i am going to read the Livongo S-1 and tweet things i find interesting. First thing (on the cover) is that they have the trifecta of big banks with Morgan, Goldman and JPMorgan. And a lot of bookrunners overall. Seems banks are pretty interested in this sector.
The Achilles heel in health innovation has always been go-to-market strategy. We are going to try to change that

While many new virtual care companies go B2B2C, we are taking a different GTM approach that we are calling B2C2B.
This is one of the best digital health proof stories in a while. Yes, apparently it is true that if we collect a ton more data on people, and use it effectively, we can diagnose diseases earlier. Nice work

Reminder that new virtual-first care companies do not need everyone to want telehealth. They only need a small % to want this mode at first (growing over time) in order to build big businesses. Retrofitting existing practices with telehealth is much harder. Thanks for listening.
This is so dumb. Swag costs maybe $1k for a small/mid company, which is not really relevant in context of total spend. But it is fun, builds pride in the company, makes employees smile and feel connected. Startups are hard and these little things help, especially when remote.
Love this. This is right in the space I’ve been thinking a lot about. Primary Care networks focused on specific groups of people with specific needs. Much more possible nationally/virtually than locally/physically.
We need more thoughtful, long form, opinionated writing in digital health, like the two pieces by
@nealkhosla
this week. Been a while since i read something that made me think about what I think (and believe). Was refreshing! Nice job Neal.
UnitedHealthcare going all in on *One app to rule them all*. 🤔
"The goal, he said, is to ultimately build a “one-stop shop” ... integration with wellness programs, claims data and remote patient monitoring are likely on the horizon."
1/ i expect virtual clinical operations // "telemedicine operations" to become a tablestakes functional area in digital health with a high demand for talent over the coming years
Sad epiphany about America. If masks were about saving the person who was wearing it, vs helping reduce spread and harm to others, everyone would be wearing a mask. Should have messaged it differently from the start.
Have to be honest. i would be really interested in fewer 'we are going to do this moonshot' type announcements from Verily and more 'we've actually enrolled some people and have learned some things from actual users' announcements.

@chrissyfarr
Oak Street has 159k at-risk patients, $2.18B in run rate revenue and market cap of $5.4B. So that is $13.7k in rev/at-risk patient/year and ~$34k in MktCap /at-risk patient. I like benchmarks.
"This was the year we saw Amazon purchase PillPack, Best Buy purchase GreatCall, Roche purchase Flatiron Health, and, just recently, Resmed purchase longtime digital health incumbent Propeller Health."
This is a big deal and much respect to the WellDoc team. These are very hard deals to get done (trust me), and now I hope the Lilly team commercializes the shit out of it for them.
@Welldoc
are the originators, and i wish them much success.
We are excited to be included in

It is only an EMR if it is fully featured, Certified and from the Verona region of Wisconsin, otherwise it is just a fancy care management platform 😂
There is so much good thinking and discussion about virtual-first care going on right now. So I wrote down some of my thoughts, and why I am so excited about this next wave of innovation. Tools are there, let's see the creativity flow!
Almost a year in post-acquisition and zero turnover from our senior team. We all like working together, ResMed has been great, and still have much work to do. Good interview with
@dvansickle
.
Couldn’t agree more. And only slightly biased! But there is a reason that all of the surviving / successful digital health companies are disease or use case specific. Building something for everyone does not work yet. Too early. Too hard.
Big takeaway from
#hlth19
: investors seem to be flocking to more focused, “niche” businesses that serve a specific population, condition or region. It’s okay to do one thing well & in health care, that can still be a large market.
We are starting to build out our team
@MyMarleyMedical
and are looking for our first engineering and product hires! Want to start from a blank piece of paper and reimagine healthcare delivery? Or know someone who does? Ping me! We are a fully remote co.
The US Gov ordered 1 billion Coronavirus test kits from Abbott, Roche and … iHealth. This has to be the greatest pivot and deal in the history of digital health. Do they still even sell blood pressure meters? Amazing.
@chrissyfarr
Yeah that’s not how it works. It doesn’t matter how much you believe it works. Only matters what you are able to prove. A better question is how do you actually create a good control group for a digital intervention?
