I judge myself on what my 8 and 80 year old selves would think. CEO of @CuraiHQ, an AI-powered virtual clinic on a mission to improve access to care at scale.

For those of you trying to intuitively understand test time compute and why it's a big deal. LLMs are next token predictors. They take a bunch of words and predicts what comes next. This is a "greedy" algorithm that predicts one token at a time - which turns out to be powerful,

one of the things that is so great about this ACCESS program from CMS is how non-prescriptive it is. we have a habit in healthcare of overprescribing the exact requirements for something rather than letting the best models emerge.

i am surprised that we haven't seen a lab try to figure out how to build a similar reasoning model with 10x lower latency. it would meaningfully increase the applications that could use such models.

i always find posts like this so funny like just simply don't invest if you think the price is bad. founders are setting themselves up for failure too. and so are the investors. so a bunch of people are gonna lose time and money - good for you for avoiding this.
The status quo of pre-seed rounds right now appears to be: > company accepts 1st VC offer > next day the company doubles the cap that said VC invested on > remaining VCs get fomo and pony up the cash at the higher val All VCs know this is the game, but participate in it anyway

the NYT article bashing it when they make statins OTC only for them to go and save millions of lives is going to be *chefs kiss*
We need more drugs to be “over-the-counter” (no prescription required). ✅ More price transparency = more competition & LOWER DRUG PRICES ✅ A prescription is necessary for some meds, but for others that are safe, the Rx just adds cost and inconvenience. ✅ FDA will be

We are not that far from people realizing that all autoimmune diseases are fundamentally more similar than different and should be treated as such.
Epstein-Barr Virus, which is a trigger in certain B cell cancers, and has been previously implicated in multiple sclerosis, is now believed to be a causative factor in Lupus, raising the chance that we could reduce disease incidence through a vaccine. https://t.co/WgxSXs2xO5

there's a lot of alpha in building exceptionally good products, particularly in AI. and the primary issue continues to be the general focus on great demos/raising more/ramping sales faster. while ignoring most of these products suck/lack depth. go slow to go fast.

average typing speed is 40 wpm. people are so incredibly slow at typing. this is a big design flaw with any chat based interfaces.

there are 3 types of VCs: 1. those w/ good taste 2. those w/ bad taste 3. those who are hoping to stumble into the big thing the josh colossus interview demonstrates how much alpha there is in knowing what you're looking for

One of the most underappreciated secrets to Silicon Valley's success is cost advantage. It was fun to unpack this with @jaltma on @uncapped_pod.

Prototypes ≠ products. Hot Vibe Code Summer is over. Woz has raised $6M to deliver real products, not just prototypes. @cervinvc led the round with participation from @ycombinator @untappedvcLA and Burst Capital. Every app built on Woz follows the same standards as a serious

the essay "the bitter lesson" has itself become a meta case of the bitter lesson folks have come to take "the bitter lesson" for what it literally says, rather than recognizing the generalized mathematical principles that inform it

here's what leads to no: 1. risk aversion 2. internal tech teams playing defense 3. competing priorities w/o clarity on where you'll accept tradeoffs 4. indecisiveness 5. allowing middle managers to say "thoughtful" things 6. falling asleep at the wheel re: tech
