Some of the biggest Claude Code fans are running it continuously in the background, 24/7. These uses are remarkable and we want to enable them. But a few outlying cases are very costly to support. For example, one user consumed tens of thousands in model usage on a $200 plan.

When doctors get to this point I always encourage them to spend more time auditing services, people, vendors, tech, coverage, etc in order to come up with creative strategies to take advantage of the current locations and panels they have vs defaulting to adding new ones.

My practice / business has gotten to the point where I only see patients 2 days a week and now I have 3 days of pure admin time. It feels so good to not wake up and grind it at 4am to do admin tasks as I grow and scale my business. It only took 6ish years to get here haha.

If longevity and wellness practices love wearables so much why aren't they being built directly into the practice like RPM/RTM?. There are so many options available that it just makes sense to bring it in house for a closer and more integrated monitoring experience.

This would allow customers to have way more control in their experience where they can upload photos to track progress, set challenges with friends, etc. Btw, @EasePractice we call this model "Services+" where doctors are able to stack D2C services on top of their core models.

Instead this service would be attached to a doctors practice vs an AI doctor (not sure what that is exactly) where meals are not only curated in collaboration with clinical professionals BUT offer the same DTC convenience where customers receive meals directly at their doorsteps.

What I mean by target goal is something like weight loss, weight gain, fitness, diabetes management, wellness, etc. By general consumer food service products I mean apps (i.e. UberEats) that are cooked by random ghost kitchens with no health data integrations.

I've been seeing more folks vibe code these types of tools where I think the timing actually makes sense to build an UberEats meets ready-made meal prep service app. But here's the twist. It has to start with a target goal and cannot be a general consumer food service product.

Iโ€™ve started taking pictures of nearly every meal I eat to feed into a multimodal AI doctor that has context on my wearable, how Iโ€™m feeling, blood work, meds/supplements, genome variants interpreted with a reasoning model, etc. Itโ€™s a very hacky prototype now but Iโ€™m surprised.

This is why from day 1 I relied on myself to survive, outlasting nearly every newcomer. Some of the people I worked with early on felt like I should have played the game, but I saw it for what it was and kept building my way. Nearly a decade later Iโ€™m still fucking here!

So much shit about founders seed strapping and how itโ€™s not fair for investors but I feel like there should also be terms that if an investor directly invests in a competitor, at a minimum their rights to information is voided and should be removed from the companies cap table.

This happens too frequently and doesnโ€™t get discussed often enough. Founders have enough to worry about, but their investors funding a competitor shouldnโ€™t ever be one of them.
