That’s the problem with different definitions. The unique difference to what is described as CME vs. a ‘conventional’ right hemi is the D3 lymphadenopathy. The other principles are principles of good surgery!

A very good question indeed. But the issue is what you define as mass closure? That is not standardised. Traditionally “mass” closure involves muscle. Therein lies the problem. The KEY is fascial only closure. I’m not convinced the choice of suture has as much impact...

I accept there is some comfort in having a firm, however that is not the reality in most places. Also there’s a certain training advantage of working with different consultants. Shouldn’t take that long to decipher what level your rotating trainee operates at.

I couldn’t agree more about technique. Technique IS imperative. The small bite technique forces you to isolate the fascia and meticulously place your sutures. You’ve taught me that very well! I’m just not convinced (yet) that the choice of knots/suture or bite size matters...
