@Amanda_M_Hunter You can add the Phso to that list .. they have this going on at the moment if you have made a complaint to them in the last 5 yrs .. although again it looks like the student marking his own homework .,but still worth putting something forward if you can .

I had a case open with PHSO for over a year which they say theyâve extensively investigated. it got closed recently, with a wholly unsatisfactory response, which basically boils down to âshe has EUPDâ. which sucks. even more so because I donât have EUPD diagnosed 𤯠#TraumaNotPD

@PeterWalshAvMA Trouble is the NHS doesn't care about patient safety. There are inadequate signals, & monitoring. Each Trust has different rules & procedures. PHSO is useless. Repeated issues may only get resolved in one small area. LCP banned in 2014 still continuing. Where are the reports?

Sounds weird!? But obviously it should do; whether itâs covered up or not I canât say. One bit of advice, donât go to the PHSO about it. They will allow the hospital to delay and put you out of legal time, as they did with two cases over my Motherâs care (same hospital)
@LorraineNorton What about an unconscious, critically ill patient who could not consent, who had a neurosurgical procedure performed by somebody who was not a neurosurgeon, nor in possession of any medical or surgical qualifications? Does that cause any legal or ethical problems?

The PHSO has a seperate function to the court re Maladministration and as you can see below it was well understood even by the DWPs own charter that DWP have a common law duty of care and that has been breached by the systemic failures evident from the material evidence provided
