63 y AAM, H/o HTN, became SOB suddenly while working in yard & by the time got to ER had to be intubated. ARF set in. TEE as below. CT Surg consulted for MR repair. Complicated course eventually Surgeon refused case on day 26 ;STS 43! Flail gap 9 mm, redundant anterior leaflet.

67 y/o with skin MRSA infection that infected his mitral valve prosthesis. Persistent bacteremia. Large vegetation on the LV side of MV. Retrograde F18 AngioVac to the rescue. Will be interesting to see long term results of such strategy. Valve with normal function.

1/4 Just publ in the Lancet our updated network meta-analysis of imaging-guided (OCT or IVUS) vs. angio-guided PCI across the spectrum of CAD. With 22 RCTs and 15,964 pts this is the 1st study to demonstrate that imaging reduces all-cause death (by 25%), as well as all MI (17%).
