Brilliantly articulated by @hjluks this is what we see everyday in Physiotherapy. Joint and tendon pain predominantly affecting people with subtle metabolic factors. I believe these MSK pain conditions should encourage clinicians to the look at the deeper causes.

In 1992 Peter Ratcliffe received this rejection letter from Nature. His findings were not "a sufficient advance in our understanding". 27 years later he won the Nobel Prize for the same discovery. Don't lose faith in the things you believe in.

🦶🏼 Medial ankle impingement PM ankle impingement T/N joint Deltoid ligament (5% of all ankle sprains) Navicular stress # (N-spot) Tib post tendinopathy STJ Spring ligament Os naviculare (at least 20% of us have this) FHL tendinopathy (often mis-diagnosed as plantar fasciitis)

'Cowboy Walks' are a great exercise for ankle strength and stability. I use them for patients in later-stage ankle sprain rehab and for Peroneal Tendinopathy. They also tend to challenge the Glutes, especially if you take bigger side steps.

Sesamoiditis or FHB tendinopathy? 🤔 Mini thread & case study 🧵 1/5 The sesamoids live within the FHB’s distal tendons & articulate with the head of the 1st metatarsal, acting as a fulcrum to increase the leverage of FHB/FHL. But is all ‘sesamoid’ pain actually the sesamoids?

Painful conditions like osteoarthritis share common risk factors with many co-morbidities across all of the bodily systems. We need to move beyond seeing pain as a "joint" or "muscle" or "bone" problem and see it for what it is...a systems problem. https://t.co/mJrm3tLpSS
