Brilliantly articulated by @hjluks this is what we see everyday in Physiotherapy. Joint and tendon pain predominantly affecting people with subtle metabolic factors. I believe these MSK pain conditions should encourage clinicians to the look at the deeper causes.
Many cases of adult joint/tendon pain are just another downstream manifestation of metabolic disease. So... Have we been looking in the wrong place? I’ve been a surgeon for over 25 years, and the longer I practice, the less I’m interested in structure — and the more I’m drawn

The war of art is one of the best books I have read. Love this from Steven Pressfield
The seriously negative consequences of not pursuing our higher calling, and the incredible consequences of when we do. What’s your higher calling? The thing you are most resisting to pursue. Steven Pressfield on the Huberman Lab podcast out now.

In 1992 Peter Ratcliffe received this rejection letter from Nature. His findings were not "a sufficient advance in our understanding". 27 years later he won the Nobel Prize for the same discovery. Don't lose faith in the things you believe in.

Does inflammation play a role in tendinopathy? 🤔 We had moved away from it but recent research is suggesting both local and systemic inflammation may be part of the pathology. What do you think?

There is a circadian program in #tendon repair. This Human study shows #tendinopathy disrupts #circadian rhythm. Implication - improving rhythm may help with recovery from tendon injury. @JPhysiol

Very clinically useful Thread
Sesamoiditis or FHB tendinopathy? 🤔 Mini thread & case study 🧵 1/5 The sesamoids live within the FHB’s distal tendons & articulate with the head of the 1st metatarsal, acting as a fulcrum to increase the leverage of FHB/FHL. But is all ‘sesamoid’ pain actually the sesamoids?

Banded side-steps are a good challenge for Glute Med and Min, especially if the band is around the forefoot.

Totally agree
Painful conditions like osteoarthritis share common risk factors with many co-morbidities across all of the bodily systems. We need to move beyond seeing pain as a "joint" or "muscle" or "bone" problem and see it for what it is...a systems problem. https://t.co/mJrm3tLpSS
