What should change: - No RCT explored pCR and surgical outcomes with a timing >12 weeks! An exception (almost) is the ongoing RCT TiMiSNAR comparing 8w and 12w (min) https://t.co/4kOzn4ugWR @IgorMonsellato Our TIMING study justify the equipoise of a >12w timeframe after SCRT !!

What was new: - >12 w between SCRT and surgery showed significantly improved pCR rates (OR 4.99 !!!) with similar surgical outcomes.

We found out that: - Delaying surgery (>12weeks) did not increase surgical complications after NAT differing from the recently published CRONOS Study (10.1001/jamasurg.2023.2521) - An optimal time to surgery after nCRT was not associated with pCR or any surgical outcomes.

This new study explores the optimal timing of rectal cancer resection after neoadjuvant therapy (NAT) to maximize oncological benefits while minimizing perioperative risks. Data comes from the largest national cohort for colorectal cancer in Italy (81 centers; >18000 pts).
