In F2 I needed a QIP, and instead of the usual VTE crap I decided to do something useful. The state of our doctors’ offices had always pissed me off and so I went about improving things, new chairs, more computers, more phones etc. Small things but made a big difference! 😎😎😎

No joke: As F2 I was once tasked with calling local zoos to see if any of them had an animal MRI scanner large enough to scan a patient who needed an MRCP who exceed our scanner’s maximum diameter
Hi I’m Elisha the F1 Doctor. My jobs today included chasing an MRI scan report from 1997 and staying on hold to the GP for 30 mins to find out more info about a patient only to be told it’s not needed and he can be discharged safely 🙃🤡

Sepsis tea trolley, sepsis lift, sepsis ambulance… all this sepsis yet most DGHs don’t have 7-day (let alone 24 hour) MRI for cauda equina… patient presenting on Friday afternoon might have to wait till Monday for their MRI… potentially debilitating… deeply unserious country

Yesterday I saved a patient about an hour in ED by wheeling them myself to X-Ray and back (porters v short-staffed)… they needed a few XRs so took ~ 30mins in total… surely not the best use of doctor time or NHS money (locum shift so ~£50/hr)… typical NHS “efficiency” 🤡🤡🤡

At my last trust when I was BMA rep I used to send out joke emails to the F1/F2s on every Bank Holiday reminding them they were the lowest paid in the hospital on those days. Even did a special one for the coronation 😅

Especially when a lot of the time pressure on doctors is due to hospitals not wanting to fund longer opening hours for the pharmacy/dispensary… it’s not my fault that pharmacy closes at 3 on a Sunday and surgical ward round only finished at 2 due to being delayed by laparotomy…

There was also a disused room which was used as a storage/dumping ground for all sorts of rubbish which I cleared and converted into an extra doctors’ office, quiet and away from the noise of the ward. Twas very well received!

Imagine a round of junior doctor strikes where PAs are “GMC registered” and have prescribing and radiation rights… would pretty much allow most things to continue as normal and completely undermine our bargaining power… patients wouldn’t know the difference until a PA fuckup…

Introducing oneself simply as “specialty” is one of the biggest red flags to look out for IMHO 🚩🚩🚩. Not just on the phone, in a previous trust I’ve seen PA documentation titled Surname (Specialty)… you have to get to the very end to clock it’s a PA.

Without wishing to brag too much… it took a lot of work! More computers required contractors to come in and install extra Ethernet and power sockets. To their credit, once I showed them my plans, our IT department was super helpful, even if they did think I was a bit obsessed 😅

And should face consequences when shifts routinely go unfilled, like every remaining doctor on shift getting paid bonus rates for covering the gap e.g. 4 doctors with 1 gap means every doctor doing 25% more work so should get 25% extra for the shift

These are complex, comorbid, often polypharmacy patients and it’s not fair that the most senior person looking after their “medical” issues is often the General Surgery F1 or F2. Completely appreciate that medics are busy as is but Gen Surg is not right for these patients (2/2)

Can we please talk about how ugly some of our hospitals are??? Obviously a lot of the Victorian-era ones are run-down etc but I suppose at least they have some character... these soulless, brutalist, concrete boxes from the 60s are the absolute worst. NW London particularly bad.
