This editorial is “a call to action”for early diagnosis of ATTR. Classic imaging (Echo, CMR, PYP) biomarkers are positive in advance phenotypes as the sparkling, shining hearts with apical sparing GLS. Need to dx ATTR in absence of classic features.

Limitation : echos were retrospective, not dedicated, standardized TR assessment, CMR prospectively obtained with analysis by dedicated researcher and radiologist. Echo core lab cannot make up for images not obtained.

Happy to see HV Doppler from @argulian . Diastolic flow reversal indicates high RA pressure. Interesting though it happens with expiration. Etiology of high RA pressure? Helpful to see medial e'. See HV Doppler of various etiologies from my Feb 2020 tweet below. @AJamilTajik.
