We hear a lot of criticism of the role of industry in medicine, but the ingenuity the space brings is extraordinary. Every time a new development from a non-cardiological field spills into my timeline, the progress feels like science fiction. This week? Weekly insulin dosing.
Weekly Fixed-Dose Insulin Efsitora in Type 2 Diabetes without Previous Insulin Therapy. once-weekly efsitora, administered in a fixed-dose regimen, was noninferior to once-daily glargine in reducing glycated hemoglobin levels.

Interesting thread. Recently had an interesting scenario. If someone is on norad for cardiogenic shock, can any of the ‘big four’ heart failure oral/NG meds modify outcomes?.What does MedTwitter think? Should we introduce ACE/MRA/SGLT2 before weaning pressors? B-blocker seems.
1/We should consider the additive benefits of #GDMT during MCS for #Cardshock. Recently, we looked at the effect of implementing a protocol for aggressive GDMT uptitration during @jjmt_heartrecov Impella support. There is a free link (only 50 first) to the publication in the end.

Sometimes, in our medical silos, information from another speciality breaks through the firewall and the advances sound like science fiction. Extraordinary progress in HIV.
Prevention beats cure, which is why it's so exciting that Lenacapavir, a twice-yearly HIV preventative, just got approved for the first time (by the FDA). Why is it such a big deal?. When trialled (in women in South Africa and Uganda) there were 0 HIV infections. 🧵

Supra-annular valves looking pretty good in particular.
The European Valve Durability TAVI Registry highlights the long-term performance of TAVI. The incidence of moderate or severe structural valve deterioration (SVD) during long-term follow-up was relatively low, with severe SVD being even rarer than moderate SVD. SVD occurred more
