Before and after. Intrahepatic cholangiocarcinoma. S6/7 remnant too small. Central hepatectomy performed (H123458-B). Luckily only 3 bile ducts to join. #cholangiocarcinoma #centralhepatectomy @WestmeadSurgery

Don't know who needs to hear this, but surgical repair of a "small fat containing inguinal hernia" for groin pain but no lump or a cough impulse is NOT going to help that patient. Please examine your patients. Do not rely on the useless ultrasound report.

How common is recurrent cholangitis after Whipple/HJ without HJ-stenosis (NSC)? And how to treat? Today in @hpbjournal 🔑 50 of 1179 (4.2%) pts with HJ developed non-stenotic cholangitis NSC. 🔑 'step-up approach' with short-course antibiotics (n = 50, 100 %), prolonged

Another Moynihan's hump just to prove it's not rare as hen's teeth and definitely something to watch out for!

@EdLinEmory @chris_nahm @WestmeadSurgery Definitely dilated and thick walled. Would do IOC and probably use PDS endoloop. Had this Moynihan hump that I shared recently.

Another teaching moment during a laparoscopic cholecystectomy. Moynihan's (Caterpillar) hump, which when unnoticed, can lead to injury to the right hepatic artery. @WestmeadSurgery

Using an exoscope camera to record open surgeries in a sterile manner, just like we do with laparoscopic and robotic surgeries. Nice technology to enhance visualization for the team and record videos of complex Hepatobiliary and Transplant resections and reconstructions.

Every case can be a training opportunity for fellows, surgical registrars and SRMOs without compromising patient safety. The Whipple's procedure is the perfect multi-step case for teaching trainees of all levels.

🧵regarding 'instrument tying' of knots. Instrument tying is a basic technique of tying surgical knots that is often used as an alternative to hand tying (which we've covered in previous 🧵). As usual, we'll go the basic technique and also look at some other caveats. (1/ )
