What is the role of pre-op RT in esophagus/GEJ cancer post ESOPEC & now MATTERHORN? I think we all agree pre-op RT not needed for all, but here is my take on 5 settings should still consider. Key is patient selection, weighing risks of local vs. distant recurrence. @OncoAlert

I’ll go beyond that and the probability that RTOG 1112 SBRT offers better survival than joint SARAH and SIRveNIB TARE Y90 is 97-98%
This is amazing and of critical importance for all fields- rad oncs control the dose , Y90 doesn’t , the meaning of small pockets of high dose distributed over a slower rate is also not efficacious - there’s a reason why there are 6 negative phase 3 trials - until dose uniformity

@SprakerMDPhD who were the responders? IMO: People will only know about “satisfaction” at least after 2 full years being in the position. So the best would be collecting data in the beginning of PGY4/PGY5 and then end of second year after accepting the position. There are also at least two

i hate doctors so much you literally have to manipulate them into doing their jobs. i’ve found doctors take me seriously when i act like i am unintelligent and uninformed. they don’t like patients using medical terms. only they are allowed to know those words

This data is useful but does not completely address the concern as none of the participants have actually entered the workforce. Sounds like there will be an “early career” survey of people who have accepted *and* worked in the job. Will be extremely valuable.

Radiation dose in SBRT vs. Y90 in HCC. This confused me for the LONGEST time. Both measured in Gy, but clearly not the same – Y90 doses often reach 400Gy+, SBRT doses much lower (<50 Gy), yet both efficacious. Explained below. Bottom line is Y90 Gy ≠ SBRT Gy. @OncoAlert
