Great case and question. I’d recheck your transducers! LVEDP from 40 to a PCWP of 5 post PCI. Highlights the importance of accurate data. If low filling pressures confirmed, check for bleeding while volume resuscitating. Lactate is the troponin of shock so monitor for washout.

This is SCAI A (at risk for shock). We do not understand ADHF/DHF/HF vs shock in

. For this reason the
#CSWG
launched a parallel registry for ADHF without shock. The inclusion criteria are: congested, normotensive, and no evidence of hypoperfusion.
