Modulating networks using DBS is an effective treatment for PD – but which circuits map to improvements in individual symptoms such as tremor, bradykinesia, rigidity, and axial? A 🧵 about our new paper out now in @NatureComms @netstim_org

🚨🥁Preprint alert: #DBS Programming is currently a tedious trial and error process that involves testing many settings – each time asking the patient to perform motor tasks to see how things improve. Could we use DBS-#evokedpotentials to guide programming?

Fantastic paper and nice tweetorial - good evidence to show that early-stage DBS might slow motor symptom progression! Read this fantastic work from @MalloryHacker et al. Thank you for including me!!.

26/n As a side note but also very importantly! our reviewer comments along with several validation results lead to supplementary material with 36 figures! Oof! Special thanks to @HelenFriedrich_ @c_neudorfer and the twitterless Konstantin Butenko for making this possible.

25/n We presented and validated across 3 cohorts, a symptom-response tract library for four cardinal symptoms of PD— tremor, bradykinesia, rigidity and axial symptoms. Finally, we developed and tested an algorithm capable of optimizing symptom profiles in patients with STN-DBS.

24/n Sounds promising! In the future, algorithms such as Cleartune could reduce patient burden and harness the potential of advanced 16-contact electrodes or multiple independent current control technology, to finely steer current and modulate different symptoms differently.

23/n We tested Cleartune in a clinical setting with 5 patients prospectively enrolled by @MartinMReich1 from the University of Würzburg 🙏. In this blinded feasibility study: for 4/5 patients, Cleartune settings led to comparable, if not better, improvements than SoC!
