17/n: Cortical sites of interconnection of these tracts mirrored the gradient of that present within the STN. Modulating connections from sensorimotor cortices in DYT, from M1 in TS, from SMA in PD, and from vmPFC, ACC, dlPFC & OFC in OCD linked to respective improvements.
3/n: This massive project is condensing 4+ years of intense collaborative work & a large chunk of my PhD – needless to say we’re over the🌙 that we are finally being able to share this with you!
8/n: Here, we took a different route, combining invasive brain stimulation with high-resolution connectomics to derive at dysfunction mappings within the brain.
9/n: DBS may be ideally suited: 1) It precisely targets focal neuroanatomy; 2) it frequently results in significant symptom change; 3) it creates an ‘informational’ lesion that can downregulate aberrant information processing in dysfunctional circuits ()
12/n: These 4 disorders all present quite differently from each other – so DBS treatment targeted to the same small nucleus providing symptom relief across all of them is rather curious. This paradox, however, came handy as it allowed mapping dysfunctional circuits comparatively.
27/n: Taken together, our findings illustrate that the STN appears to contain a miniaturized representation of fronto-cortical dysfunctional topography.

11/n: Using DBS Fiber Filtering, we investigated dysfunctional circuits in 261 patients treated w/ 534 DBS electrodes generously shared in a large multi-center collaboration involving 10 specialized centers in 7 countries. Patients received DBS to the STN for PD, DYT, OCD, or TS.
Before tackling the circuit level, we first took a closer look at the target level. Specifically, we applied DBS Sweet Spot Mapping () to investigate focal stimulation sites associated with clinical improvement in each of the four brain disorders.

21/n: To further substantiate our findings, we validated tract models in two independent samples of n=32 PD & n=35 OCD patients. Notably, the OCD cohort had been implanted to the ALIC region. Indeed, overlap of stimulation volumes w/ the model could estimate clinical outcomes.
13/n: Unsurprisingly, average electrode placement within each disease cohort was comparably similar (within the posterolateral STN) across three/four disorders. The only exception was formed by OCD where surgeons are known to deliberately target a more anteromedial STN portion.
15/n: Similarly, filtering modulated neuroanatomical tracts from a high resolution group connectome by their relevance for beneficial outcome within the respective disorder revealed a certain degree of segregation in the topography of dysfunction mappings.

16/20. Relating cortical & subcortical gradients, our findings further illustrate that the STN contains a miniaturized representation of fronto-cortical functional topography. Beyond segregation, our results may thus be reconciled with the idea of spatial information compression.
8/20. We thus set out to leverage optimal connectivity profiles of invasive DBS electrodes to the STN in four different disorders (DYT, PD, Tourette’s syndrome/TS, & OCD) as a means of systematically mapping the functional organization of the STN & that of the frontal cortex.
