Careful study just out, led by @KidneydrChirag + @yumeng_wen that shows something I would never have believed not long ago -- that deceased donors who were on dialysis during their terminal hospitalization can provide kidneys that do well after transplant.

Our prior work has shown that extremely low fractional excretion of sodium ; FeNA of <0.25% (not <1% as patients with cirrhosis are <1%) is diagnostic of HRS. Combining urine NGAL for ATN and urine FeNA for HRS can facilitate management in cirrhosis. @scoca1 @hswapnil

In conclusion, urine NGAL is a valuable tool for differentiating ATN in patients with AKI and cirrhosis. Future studies should evaluate real-world impacts of NGAL testing and establish guidelines for its use in various clinical contexts. @VelezNephHepato @JenZ4NGAL

Limitations: 1) Universal NGAL cutoff may not be appropriate for all patients. 2) HRS definitions varied across studies. 3) ATN/HRS diagnoses were clinical, not biopsy-confirmed. 4) Biomarker assays and reporting methods differed.

Median optimal cutoff for urine NGAL to diagnose ATN was 220 mg/g creatinine. NGAL is approved for clinical use in Europe/Asia & recently received FDA approval for pediatric use in the US.

We included 11 studies with 1219 patients. Pooled sensitivity of urine NGAL for diagnosing ATN was 81%, specificity 82%, & area under the receiver operating characteristic curve was 0.88.
