Really proud of this work, now published in @braincomms (in MS Word form for now at least). We consider the ethical conundrums surrounding dementia research, examine the existing legal & ethical guidance, & try to plug some of the gaps in that guidance.

So what? AD is a disease of arousal dysfunction. This represents a plausible link between sub-cortical (including LC) pathology and cognitive symptoms. Arousal dysfunction is quantifiable using neuroimaging (EEG, MRI, pupillometry), and represents a therapeutic target.

Using an oddball task (surprising stimuli in a sequence of repeated stimuli) to elicit phasic arousal responses, we found the AD group to again show dysfunctional (reduced) arousal activity. This again related to cognition (ADAS-Cog)

In general, these very strong relationships are preserved in AD, but with some alterations. For example, higher pupil-linked arousal, the faster the alpha rhythm, in AD but not in healthy older controls.

Pupil size gives a readout of arousal level/LC activity. Pupil size strongly correlates with EEG power (cortical activity) during rest - they are tracking a shared phenomenon - tonic arousal!

The biomarkers of neurophysiological arousal include the peak at which the alpha frequency occurs measured using EEG (higher arousal tone, higher frequency). Lower frequency = cortical slowing - an established finding of AD, that appears to be related to arousal/LC-NA dysfunction

The locus coeruleus (LC) is the principle source of noradrenaline (NA) in the brain and a key arousal centre. Due to tau accumulation, the LC-NA system is dysfunctional in Alzheimer's (AD). This impacts the neurophysiological phenomenon of 'arousal' in AD, affecting cognition.

Some prevalent symptoms (particularly psychiatric) that relates to concussions, but overall reassuring that 0/200 have dementia (mean age 44y/o). However, potentially concerning evidence of early pathology (raised ptau217, frontal/cingulate atrophy).
