My thoughts exactly. He’s probably getting convalescent antibodies today and would get remdesivir/dex at earliest possible indication of progressive lower airway disease. His course will absolutely not be representative of what the public at large faces.
Academic medical centers are headed for a staffing crisis. Too many faculty are burned out, feel unsupported, & there are other options. I’m not going anywhere, but have heard about too many w imminent changes afoot to not be shook. And that’s just the docs. RNs are bailing too.

Not so fast. Spleens exist to satisfy hematologists — by recommending their removal. Appendices exist to give surgeons a starter procedure to practice — so they can grow up do the big ops, like laparoscopic splenectomies in patients with platelet counts in the single digits.

10yrs ago I paid $$$$ to take the ABIM Hematology Boards with the understanding they’d be good for 10yrs. Now, to recertify 10yrs hence, ABIM is charging me MOC+overdue fees for the PAST 5 YEARS, in addition to the recert test fees. Honest Q: How is this mob-like behavior legal?
I’m at a juncture where I’m asking for advice from many colleagues across the country. One thing I love most about hematology (prob true in other fields) is the implicit camaraderie when you reach out to someone & they give their time to offer truly valuable guidance.

I just wanted to point out that you can see in the video, and from the pattern of redness on her face, that she was wearing a mask when sprayed, but undoubtedly had to remove it after it was covered with irritant. Maybe she just didn’t have a second mask with her.

Rezvani: NK cells offer advantages over T cells for CAR therapies, but there are disadvantages such as limited numbers in circulation and limited persistence.
A career in BMT/CT is richly rewarding, but what we do, especially alloHCT is very specialized and legitimately requires additional training beyond standard Hem/Onc training. Patients deserve competency. Competency comes from adequate training and exposure. That’s just a reality.

I’m already in the mask for life camp (anywhere but outdoors), because I’ve enjoyed not having any URIs for the past year and a half, but with a vaccinated colleague testing positive for COVID today and having pretty significant symptoms, I’m not regretting my choice.
Completed a 12mile trail run today. Exhausting but exhilarating! Clinical medicine is like that. Exhaustion coefficient is high currently. Lots of friction to get things done. Lots of burnout (which isn’t a sign of weakness or an individual’s fault). Still best job in the world.

I no longer transplant Ph+ B-ALL pts if they tolerate hCVAD+ponatinib and achieve CMR. Ph+ B-ALL should no longer be considered high risk when treated with this specific regimen and fewer patients should be subjected to the toxicity of alloHCT. This is the way.

I took my hematology board recertification exam yesterday and I don’t want to alarm anyone (or violate the confidentiality agreement) but I think ABIM may be trying to prepare us for a looming pandemic of congenital bleeding diatheses heretofore so rare I’ve never seen them.
Ended a long week with a 4hour NCCN call, then 4.5hours UCSF Hem/Onc research symposium, and then a death-defying float in the SF bay at night (with a broken toe). Lost my nerve a little when something bumped the bottom of my hull in the darkness… 🦭 or 🦈? 😳
