Our findings therefore suggest a unique immunology of psoriasis and eczema on acral sites – this may underlie the often-overlapping morphologic features seen in these conditions as well as their incomplete response to biologics that target a single cytokine pathway. (7/n)

However, we were surprised to find that IFNG and IL13 were both expressed in PP as well as HPE. This contrasted with the divergent expression of these cytokines in non-acral psoriasis (IFNG predominant, no/low IL13) and non-acral eczema (IL13 predominant, no/low IFNG). (6/n)

We found that IL17A mRNA staining distinguished PP from HPE, similar to how IL17A expression distinguishes non-acral psoriasis from non-acral eczema. (5/n)

We evaluated expression of IL17A, IFNG, and IL13 in acral biopsies using RNA in situ hybridization (RISH). Our lab previously showed that RISH for these cytokines effectively distinguishes psoriasis (IL17A+IFNG) from eczema (IL13) on non-acral skin: https://t.co/DoYKehSgZ5 (4/n)

Our goal was to determine whether cytokine biomarkers could augment diagnosis of acral dermatoses such as hyperkeratotic palmoplantar eczema (HPE) and palmoplantar psoriasis (PP), which can at times be difficult to classify clinically and histopathologically. (2/n)
