Does not have to be all or nothing. One option would be to only digitise biopsies (smaller digital footprint) but not resections. Have to consider the costs (not just monetary) and the potential benefits.

An example: NCCN risk stratifies prostate cancer based on % core involvement. How can 3mm tumour in a 5mm core (60%) predict worse outcome than 10mm tumour in a 20mm core (50%)? Clearly needs more path input.

Making a diagnosis is not the end of the diagnostic process. Need to properly communicate this information and help clinicians (and patients) understand the issues in complex cases. Unfortunately, many (most?) clinicians do not really understand histopathology.

Agree but only if pathologists adopt a patient-centric (rather than specimen-centric) approach. Pathologists need to up their game, subspecialise and be more closely involved in patient care. Personalised Medicine requires Personalised Histopathology Reporting!
