A RCA fistula to the LV is a rare anomaly where blood flows abnormally from the RCA into the LV, bypassing myocardial capillaries. This creates a left-to-left shunt, increasing LV volume. Large or symptomatic ones typically need closure to prevent long-term complications. ❤️🩹
