Great session moderating today’s @SCMR education series on aortic valve disease. A nice tidbit: assessment has moved beyond just LV size/function. T1 mapping & LGE let us assess myocardial health in valve disease and with prognostic value earlier—before dysfunction sets in.

The @US_FDA approved updated #COVID vaccinations . The guidelines for approval are controversial given the limited patient scope. But one thing is clear - every single adult with repaired or unrepaired congenital heart disease is eligible for vaccination. #ACHD ❤️🩹.

Unroofed coronary sinus with persistent LSVC is rare but important to recognize. If you inject agitated saline from the left arm and see bubbles enter the left atrium via the coronary sinus, suspect this defect. It’s a clue you don’t want to miss. #ACHD ❤️🩹

Teaching moment everytime: If you see a moderate-sized secundum ASD on echo and there’s RV dilation, you don’t need a cardiac MRI just to calculate Qp:Qs. RV dilation = hemodynamically significant shunt. That’s the definition. #achd #echofirst ❤️🩹

Cleft mitral valve has always been a good reminder to me as to how structural problems can lead to physiological problems. Sometimes the GDMT people need is a surgery. Not this case specifically, it’s only moderate. But in #ACHD it happens more often than not. ❤️🩹

Parallel course of the aorta and pulmonary artery on the parasternal long axis view suggests transposition of the great arteries (TGA). Normally, they cross at right angles — parallel flow is a red flag for abnormal spatial relationships! Note also the VSD! #ACHD #Echofirst ❤️🩹

Patients with a dominant RV after a Fontan may exhibit a greater decline in systemic ventricular function over time. This decline becomes more pronounced after the first 10 years postop compared to those with a dominant LV; though in this adult it still looks pretty good #ACHD ❤️🩹

The Fontan procedure is applied to various single-ventricle defects. Advances in surgery and postop care have significantly improved survival rates. Estimates suggest that over 70,000 patients worldwide live with Fontan circulation and expected to double in 20 years. #ACHD ❤️🩹

Preventive cardiology should be part of the annual visit for ACHD pts. Mitigating risk factors like htn and dyslipidemia reduce CV events. Screening for Lp(a) and future treatment will go along way to decrease the burden of CAD in this population #ACHD

Nice example of an extra-anatomic graft for coarctation of the aorta. Surgical correction of complex aortic coarctation can be associated with significant risks. Extraanatomic bypass can be used as an alternative to a higher risk repair. #ACHD ❤️🩹
