Anxiety and muscle function are both recognised causes of breathlessness. Mediation analysis revealed that these factors could mediate some (18-41%) of the association between sleep disturbance and breathlessness.

Sleep Quality, Deterioration in sleep quality after COVID-19 hospitalisation and lower sleep regularity were all associated with breathlessness, lower lung function and reduced SARC-F scores. Whilst subjective sleep metrics were also associated with anxiety.

Device-based metrics revealed that participants slept on average 65 minutes longer, had lower sleep efficiency (3.8 percentage points) and a 19% reduction in their sleep regularity index compared to a matched UK biobank cohort who had been recently hospitalised. Blue=COVID

The findings were broadly consistent regardless of what type of measurement was used. Firstly, sleep disturbance is common in post COVID-19 syndrome with 62% of participants reporting poor sleep quality at 5 months. This is likely to persist for at least 12 months.

Our study investigating if sleep disturbance is associated with post COVID-19 breathlessness published yesterday in Lancet Respiratory Medicine will be presented at ECCMID today. This large multi-centre study used both subjective and device-based measures.
