And to be clear I agree. I think guidelines are mostly unhelpful for selecting therapy because of the limitations in clinical trials for selecting the right drugs for the right patients. NMAs should not be used to tier therapies especially by reimbursement bodies.
Prof Peter Irving elegantly describes the approach to selecting therapy in 2025. And in the process politely "trashing" guidelines and Network Meta-analyses :). One take home is early IBD responds better and if we treat early prior to complications it matters less which therapy
