Everybody wants the benefits of good incentive structures, but no one is willing to suffer the costs of good incentive structures. So we get bad incentive structures and people whining they can't get what they want.

Sure. By increasing the cost of foreign consumer products, production moves onshore. This increases demand for domestic labour, raising wages for Americans, while also raising tax revenue, naturally redistributing money from foreigners and the rich to the lower classes.

You can train enough American doctors. The spots for training doctors are purposefully limited. Just make it illegal for the AAMC to restrict training and residency slots and you can fix the problem without importing foreigners.
