The cure for most cases of type 2 diabetes (T2DM) requires fat loss. This thread will discuss research showing restoration of carbohydrate tolerance in those with T2DM through addressing the underlying pathology (ectopic fat) rather than just focusing on symptom management.

I use this platform primarily for discussion and a way for me to formulate, refine, and communicate my beliefs. When I quote someone, it’s because their post raises an idea that deserves a closer look, a rebuttal, or some added context for everyone following along. I don't care

I agree with the push for better precision medicine. I think such a future will be invaluable. But the way NNT is being used in this post is a gross misrepresentation. NNTs for the top 10 drugs come almost entirely from short trials, usually 2 to 5 years, while people live with

Energy surplus is the proximal cause for obesity. Calorie intake is a variable we can manipulate with great precision to cause predictable changes in body mass. The practical utility of CICO is debatable, but the biological mechanisms are well-established.

Editor's note: babies enter ketosis rapidly due to extraordinarily high energy requirements. They require ~100 kcal/kg during the first 6 months of life, which is 2-3 times higher than competitive athletes. About 40% of breast milk calories come from carbohydrates.
"Ketosis is a starvation state! Your body only does it in emergencies!" Humans are born in ketosis. Newborn babies, literally fresh out of the womb, are in deep ketosis. Breast milk is designed to keep them there: 60% fat, moderate protein, low carb. If ketosis was an

I have a hard time believing that poverty is the biggest driver of obesity. Not just because the claim is weak, but because it promotes a kind of learned helplessness where people lack agency because of their circumstances. That mindset is fatalistic, disempowering, and helps no
Spoiler: they’re not gonna do that. Biggest driver of obesity in the US? Poverty. RFK Jr won’t address: Social determinants of health No universal healthcare No Federal living wage Food deserts Housing inequity Wealth gap They also got rid of nutrition education & assistance!

I used to think this too, but it's based on the false assumption that you can't prove a negative. It doesn't matter whether someone says vaccines cause autism or vaccines do not cause autism. Both are claims that can be empirically investigated. The burden of proof is always on
“‘Vaccines don’t cause autism’ has never been proven.” Correct, because that’s not how science works. If I say “sunning your perineum twice per day increases your IQ by 10 points” you would say, “prove it.” The burden of proof is on me, the person with a tanned perineum. If I

I think it's silly that I needed to even clarify this, but so many people in these comments are missing the point. https://t.co/WqeJKAETUU
A lot of people are completely missing my actual point. If you think formal education is biased or useless and you also don’t have that same formal education, then using its absence as an attack against someone else is logically incoherent. You can’t dismiss doctors for having

There's a whole subset of people who will criticize medical doctors like this while they themselves have little to no education on these topics. For some bizarre reason, they think that going to the gym and watching YouTube videos means that they are more knowledgeable.

Your doctor spent: 6 years learning how to prescribe medications. 4 hours learning nutrition. 0 hours learning how to reverse chronic disease. But sure, take dietary advice from someone whose entire toolkit is "here's a pill for that" while 93% of Americans are metabolically

I really like this study for illustrating the point. It shows that fat cell hypertrophy regardless of fat composition or inflammation invokes insulin resistance. Doesn't matter what causes you to be too fat. Being too fat is itself the proximal cause of insulin resistance in fat

In obesity, adipocyte hypertrophy and proinflammatory responses are closely associated with the development of insulin resistance in adipose tissue. However, it is largely unknown whether adipocyte...

I've never seen a clinical trial of GLP-1 receptor agonists that validates these claims. Most report 20-40% lean mass loss, and this is explained entirely by being sedentary and not eating enough protein.
Peter Attia: "Ozempic & Wegovy's Dark Secret - You're Losing Muscle, Not Fat" Dr. Peter Attia reveals the shocking body composition data behind the weight-loss craze. "The ideal ratio for weight loss is 75% fat, 25% lean mass. But with these drugs, we're seeing the opposite."

100%. It’s not the emotions, but how they are expressed. If a man collapses into weakness or uses self victimization language, I need to *man* up myself now. Which flips the polarity. And I can’t trust the man to contain and protect me. It’s really as simple as that.
It’s true that many women say they want a man who’s “in touch with his emotions,” yet get the "ick" when he expresses them. This can be bewildering for men, but the issue lies in how emotion is expressed, not in the fact of feeling it. Yes, women tend value a man who has

Editor's note: Long-chain omega-3s like EPA and DHA actively resolve inflammation by making specialized molecules called resolvins and protectins that switch the immune system off once the job’s done. PUFAs can oxidize, sure, but the net effect of omega-3s in humans is lower
Omega 3s don’t “lower inflammation” The reason why omega 3s appear to lower inflammation is because the body responds to oxidative damage caused by PUFAs by up-regulating endogenous antioxidants (adaptive hormesis)… not because omega 3s are inherently good. Human RCTs show

There’s a common narrative that high LDL or ApoB is merely a “marker” of poor metabolic health rather than a true cause of heart disease. It’s a seductive idea, but not quite accurate. Let's start with what Tro got right: insulin resistance, chronic inflammation, and metabolic
It can be true that LDL levels and LDL-lowering are associated with heart disease outcomes, much like fever and temperature-lowering are associated with outcomes in sepsis. Lowering fever may help a patient feel better or even marginally improve survival, but it doesn’t mean the

Editor's note: Heart disease isn’t caused by one thing. It’s the result of many interrelated factors like blood pressure, inflammation, insulin resistance, genetics, and yes, LDL particles. Lowering one causal factor doesn’t erase the others. That’s like saying, “If smoking
Remember if cholesterol was the direct cause of coronary atherosclerosis and coronary heart disease, then the absolute (not relative) reduction in heart disease risk with blood cholesterol lowering would be 100%, not 1.8%. When a lipid-lowering drug approaches a 100% (absolute)

Here's a link to my prior post on raw milk food-borne illnesses. https://t.co/1lQJDuqfnn
With all the opinions on raw milk floating around, I want to share my thoughts on what is probably the biggest reason people seem to be against raw milk — food-borne illness. I think that the risk of becoming sick after drinking raw milk is highly contextual and that it's

I wrote about the literature on raw milk safety almost a year ago, which I will link to in a follow-up comment, but I think it is also worth looking at the potential benefits. First, to summarize my prior post, the major routes of milk contamination are environmental
🚨 The raw milk trend is getting people killed. A Florida woman lost her baby and nearly died after being infected by E. coli traced to raw milk from a local dairy. She didn’t even drink it — she just cared for her sick toddler who did. This isn’t a one-off. Florida’s

I once again urge everyone to judge people on the basis of their arguments, not on the basis of their credentials. Saying somebody is not a physician to discredit them, as if that has any bearing on their knowledge, is pure appeal to authority.
A note: Peter Attia is *not* a physician. He is not a scientist. He graduated medical school. He didn’t complete residency. He is not board certified. After dropping out of residency, he worked for McKinsey. He then founded a private “longevity clinic” to hawk pseudoscience. 1/
