Now that the X-waiver has been X’ed ( https://t.co/7ka8Z1ouWq) , we have a huge potential pool of buprenorphine prescribers in EDs. We have more work to do, but we hope strategies like ours can help other EDs out there trying to do this work.
acep.org
In DEA guidance released in Jan 2023, an X-Waiver is no longer required to treat patients with buprenorphine for opioid use disorder. ACEP has more details about treatment options and training...

These results built on already high rates of OUD treatment in EDs, mainly by increasing prescribing among low-adopters. Importantly, this intervention was implemented as part of usual care with minimal support, representing real-world clinical practice.

We also saw a 5% increase in buprenorphine rx's and a 12% increase in Narcan rx's at discharge. Put another way, for every 20 patients with OUD exposed to this new pathway, one additional patient received a bupe prescription at discharge.

Breaking it down: We found that withdrawal assessments with COWS increased substantially in patients with OUD with screening intervention implementation. This is the first step in assessing patients with OUD and potentially starting treatment.

Here, we compared three EDs that implemented universal triage screening with 2 others that did not. At a high level, we found that screening was widely adopted, feasible, and associated with significant increases in the assessment and treatment of OUD.

First, we worked with ED stakeholders to design + implement a universal triage screening with automated prompts to physicians and nurses to assess and treat OUD. More on that here:
