a mobile crisis nurse reported to me crystal meth, on the streets of toronto, costs….wait for it….$3/bump, cheaper than any beer. the crisis is getting worse, contributes greatly to homelessness. open air use is common. solutions? ban pseudophedrine/ephedrine? rx adderall? +?

carbon what? who needs oxygen when you can sit in a spa! in the cover of darkness, @ThermeGroup @ThermeCanada and @fordnation showed their future-forward approach to urban redesign by knocking down a grove of hundreds of mature trees!

i love my job as an emergency physician so much. i can’t imagine another medical specialty; it’s so interesting and flexible. AND if you’re a medical nerd, by the end of a career, you’ll have seen a wider variety of cases/helped more strangers than any other type of doctor.

dear FPs/consultants, when sending a patient into the ER, don't clinically hem us in by saying "patient needs imaging" (or equiv). it creates expectations, and sometimes they don't. at least add "at the discretion of the emergency physician". but you're cool for sending a note

@jamesmaskalyk @davidcarr333 100% this. I've gotten away from just a code status and write my full goals of care discussion. What is the goal of CPR, intubation, etc. This can really help contextualize the role of these interventions and allows for more nuance than the yes/no of code status.

I graduated from my emergency residency, good god, 20 years ago. first time w @MSF just after. I like ER, and MSF for the same reason: lots of patients. human lives are just so interesting and beautiful. you want to give them every chance.

anesthesia fam. my father (79) fasted most of 6 days waiting for an "emergent" procedure. it led me to reflect on the % of people who vomited of the hundreds I've RSI'd in 20 years: zero-ish. he, however, lost 5 lbs muscle he won't get back. can we expand the notion of "risk"?

can someone help disambiguate how provinces decide how much $ to give hospitals? is part of the formula occupancy, ie, inpatient census? if so, needs to change from rewarding max occupancy to ideal (ie, 85%, above which, backlogs in ER/OR’s occur, and penalties should be applied)

“can you tie your urge to drink despite the harm to a feeling? is there an unmet need underneath that feeling…connection, safety etc… that isn’t being met? if so, i wonder if we can come up with some other ways to get to that place such that it lasts.” right? recommend.

compare “using so much alcohol is bad for you. you should stop. trust me, i’m a doctor, seen it so many times. if you don’t control your use, you’ll get liver failure, maybe even die from it. we can’t keep on having this conversation every time you end up in the ER” vs
