Revascularizing viable “hibernating” myocardium? This practice-changing trial saw no benefit with PCI, both in mortality as well as ejection fraction improvement — and tore at the very fabric of viability testing itself. REVIVED-BCIS2 Trial, NEJM 2022 ♥️

Disruptive data from @REVIVED_BCIS2 viability analysis! ❌️ Amount of hibernating myocardium NOT associated with prognosis /LV recovery ❌️ Viability testing does NOT identify patients who benefit from PCI However ➡️ scar burden closely correlates with prognosis /LV recovery

3) This trial has major implications not just for the HF & IC docs but also the imaging community What *exactly* is the role of viability testing going forwards? Here is a table comparing prior studies looking at hibernating myocardium...

Dobutamine Stress Echo, Interpreting Wall Motion Responses The wall motion response of different myocardial segments helps us distinguish: ⬜ Normal myocardium ⬜ Ischemia ⬜ Viable tissue ⬜ Scar ⬜ Hibernating/stunned myocardium. ~ a thread ~

Hibernating vs. Stunned Myocardium: What’s the Difference? 🔹 Hibernating Myocardium = Chronic ischemia leads to reduced contractility as a survival mechanism. Reverses with revascularization (PCI/CABG). 🔹 Stunned Myocardium = Temporary ischemia followed by reperfusion injury.

Case follow up from FIT Dr Hutt: 1) FDG PET w hibernating myocardium in the anteroapical LV w FDG uptake showing viability 2) Revascularization of the CTO of LAD 3) Normalized LV function 4) Symptom improvement for the patient Great job with case! @MyASNC #CVNuc #ASNC2022

Case of PET Viability with stress-induced ischemia and hibernating myocardium in the LAD territory presented by Dr. @PanithayaC from @MayoClinic. Also, note that reverse mismatched areas (i.e. -ve FDG and +ve perfusion) are viable. @MyASNC #CVNuc #CardiacPET intensive workshop

Hibernating myocardium is a condition where a part of the heart muscle has difficulty contracting due to reduced blood flow. It can occur over days, months, or years, and can be reversed with revascularization. With permission from the patient to post, this video shows the left

I think this pattern suggests hibernating myocardium. When a predominantly fixed defect has normal MFR (albeit low absolute stress MBF), the tissue is probably viable. Infarct shouldn’t be this vasoreactive. I included my suspicion in the report.

65/M S/P PTCA to LAD (2016) for AWMI Now, presented with DOE FC II Stress MPI showed small to moderate sized hibernating myocardium in midventricular region. Took up for angiogram. Was expecting an ISR and found this! #CardioTwitter

#EditorsChoice This study from Song, Fang, and coauthors aimed to examine the impact of integrated assessment of hibernating myocardium and scars on the survival benefit associated with CABG in patients with ischemic left ventricular dysfunction👇 https://t.co/Y04SIjgLQw

Hibernating myocardium indicates chronic myocardial contractile dysfunction due to ischemia, where resting blood flow is reduced. The contractile function of the involved myocardium can be restored by improving coronary blood flow or reducing myocardial oxygen demand.

Different mechanisms of acute and chronic LV dysfunction in CAD. In both stunned or hibernating myocardium, LV function is depressed, but remain reversible, because the myocytes are viable. In clinical practice, the adaptive responses of hibernation and stunning coexist.
