I get asked all the time:.“Can I stop my blood pressure meds?”.If you're BP is over 130/80, the answer is basically always no. But even if it's lower and if you're doing well on meds?.It makes sense to stay the course. That’s what the best trials show (SPRINT, STEP).

Most people don’t have a single cause of hypertension. It’s multifactorial, with contributions from:.-Poor sleep.-Too many calories (and sometimes too much sodium or too little potassium).-Too little exercise.-Too much visceral fat.-Genetics.All layered together. Lifestyle helps.

In any given week, I hear questions from about 10 patients about some esoteric health trend they heard about on a podcast or Instagram.Approximately zero ask me about their blood pressure. Skipping won’t save you from a stroke. But treating your high BP might.

Here’s the takeaway:.A low-carb diet can help and hurt. If it makes you feel better but your LDL spikes, don’t ignore that. You can keep the diet and treat the number. Those aren’t contradictory goals.

Some influencers focused on the fact that LDL and apoB didn’t predict progression in this group. But when everyone’s LDL is off the charts, you’re not testing the value of the marker. You’re seeing what extreme levels can do.

These patients had the fastest progression of coronary plaque ever reported in a group like this. Even without insulin resistance. Even with great blood pressure. Even without inflammation. Just LDL cholesterol. A lot of it.

A recent study followed people with super high LDL on a low-carb diet, using serial coronary CTAs to track plaque growth over time. They were otherwise healthy. No diabetes, no meds. But the results were alarming.

Some people in this diet camp have coined the term for these folks 'Lean Mass Hyper-responders'.They're fit, insulin-sensitive, and have normal triglycerides. but their LDL triples on keto. They feel great.Their glucose is perfect.They look healthy.But do their arteries agree?.

✅ Evidence > mechanism.Flecainide suppressed PVCs. Great mechanism. Then CAST showed it killed people. Great reminder. Mechanisms are seductive. Outcomes are what matter.

✅ Don’t let someone else do your thinking. Consults are helpful. So are attendings. But you need to think through the case first. Bring a point of view. That’s how your brain gets better.
