@elm_slippery there is a whole new bunch of self styled 'patient safety experts' trying to blame everything on system failures. works nicely for them as they get invited to deliver lectures in all these patient safety conferences whereas whistle-blowers are all completely sidelined

Systems error means no individual directly accountable ! Means no investigation by PSNI ! Did inquiry name those responsible for the cover up?

Systems failure much loved by those responsible for a hospital death . Denies individual culpability . In other words it is the system that is responsible ....LOL.

No, it is not & that is the point of the nonmedics. often there are individual failures which are almost always covered up by the healthcare establishment incl the medical profession

Have it in buckets! When the matter has been apparently predetermined battling with one oar in extremely rough seas. Extreme learning curve when you challenge the establishment.

Exactly! Problem centres around getting a court to accept an establishment figure is lying through his/her teeth
@elm_slippery Just googled. Falsus in uno, falsus in omnibus. At common law , it is the legal principle that a witness who testifies falsely about one matter is not credible to testify about any matter.

Calling all lawyers @MOReilly01 From my memory if a witness lies , what is the legal term? It casts doubt upon any further statement or evidence that they give?

Inquests must be open and transparent . If they are not there can be no learning from past errors. Wading through layers of corruption to uncover the truth. There should be no hiding places for the wrong doers.

N.Ireland Justice Committee must investigate whether joint protocols are or were in operation between the coroner and the NHS in northern Ireland . Possible reason why telephone call to the coroner all it took to stop an inquest into highly controversial hospital death.

Not referring hospital deaths to coroner.....seems may have been joint protocols in operation! To conceal a death one telephone is all it took. Problem is that majority in N.I could not care less.
Shipman also did not refer deaths the coroner. What did Sir John O'Hara confirm??! "The fact that her death was not referred immediately to the Coroner for an inquest is *indefensible* And it is not just indefensible with hindsight - it was indefensible โ ๏ธat the time โ ๏ธ (96)
