3/.Our questions:. Did the heparin prevent the stroke?.Do these clots just come and go in patients with afib and we just happened to catch one?.If the clot remained, is there a role for some sort of thrombectomy?. I don't know the answer to these questions, if anyone does, reply!.

2/.A couple of hours later, the intern on his ultrasound rotation went to perform an educational POCUS. He came to show me the images:.him:"I can't find the clot".Indeed there is no clot. Me: "Is she talking and moving?".I ran over to her room and she was not having a stroke.

1/ The disappearing thrombus. This patient in her 70s presented with SOB and was in Afib with RVR. The first clip taken by the resident on arrival shows an LA thrombus. She reported compliance with her meds but we started her on heparin after this #POCUS.

6/.pt satisfied with my #POCUS as a test, and me satisfied there was no stone, I discharged him with sialogogues and NSAIDs, with primary care follow up and an ENT referral if needed. No, this POCUS wasn't life-saving, but it did save time and reassured the patient. POCUS ftw!.

5/.And the parotid duct was enlarged, though when I followed it I could not see a stone, which would look hyperechoic with posterior acoustic enhancement, like any other stone.

2/ While this is certainly is a clinical diagnosis, I both wanted to know if there was a stone there, and the patient wasn't satisfied with my clinical diagnosis and wanted a confirmatory "test". So out came the #POCUS.

1/ #POCUS for the Parotid- Yes it's a thing. A middle aged gentleman presented with left cheek swelling and tenderness for 2 weeks. His exam was consistent clinically with parotitis.

💯. This is why POCUS courses for practicing physicians need to include clinical integration. Knowing how is only half the battle. Knowing when is the other half.

👩‍🏫 ACGME proposed that POCUS faculty just need “focused experience.”.❌ We recommend clearer standards. 📌 Qualified faculty should hold ABEM FPD-AEMUS certification, fellowship training, or equivalent—ensuring safe, expert-led teaching.
