#POCUS #MedTwitter #Nephpearls Many #VExUS enthusiasts asked for a #tweetorial on image acquisition pearls. Did one b4 but time for an updated one 🧵 #1 Let's start with basics Color Doppler identifies the flow + tells the direction (blue is away & red towards the probe [BART])

Saturday’s shift image! Lung point sign has an overall sensitivity of 66% and a specificity of 100%. Sensitivity is low because in large pneumothoraces, there is no point when the inflated lung is in contact with the parietal pleura.

#POCUS answer: 🔵 Parasternal RV inflow view 🔵 IVC Doppler demonstrating S- and D-waves -> above the baseline because the blood flow is towards the transducer in this view. #Nephpearls #FOAMed #FOAMcc
#POCUS quiz of the day ⭕️What view is this? ⭕️What vessel is being sampled? (Doppler waveform) Clue: Helpful in cirrhosis where subxiphoid window is difficult #Nephpearls #eVExUS #FOAMed

Post-paracentesis (3L): - Diastolic flow improved - Notching less prominent - Venous flow more apparent (previously low velocity continuous [not a 'good continuous'])

. @icmteaching @khaycock2 @ArgaizR Have you seen this notching/systolic spike accentuation in patients with cirrhosis? I think it's a manifestation of intense renal vasoconstriction (as in HRS pathophysiology +/- other similar insults).

@NephroP I've been thinking about this. ESRD patients often have pulmonary hypertension. In contrast with pure volume overload, these patients typically have: • A flat, depressed RV function curve (high afterload, poor reserve) • High RAP from “can’t empty” rather than just “too full”

I still see 0ּּ·9% sodium chloride being used for resuscitation, and amazingly even for maintenance fluid. Let’s take a look at one important aspect of that. The huge salt burden in just one litre.
