The past few weeks have been the toughest I can remember in 13 years as a doctor. This week felt like walking on the edge of a cliff. Every bit of the NHS and social care is creaking. We know our waiting times are terrible. We know people are spending hours on trolleys. It hurts

After a tough few weeks really touched and chuffed today to have an email confirming I have been nominated and awarded Fellow status for the Royal College. Hopefully FRCP will reflect the services we have improved as a team for patients with hepatology disorders

I spoke to mam and dad this evening over the phone... And broke down crying within 60 seconds :( I miss seeing them and it bloody hurts that I may not see them much if at all until the autumn due to shifts and lockdown.. This Covid19 really is shit and it is already breaking me

What is worrying, depressing but also fascinating about current covid data is that the hot spots on the maps almost perfectly overlap both areas of socio-economic deprivation but also the historical coal field hot beds. Lots of learning to be had after the pandemic is over
I don't think I'm a particularly spectacular doc but 4 times this week pts/families have thanked me for explaining things to them in a way they understand and for being "down to earth". Isn't this what we should all be doing? Ditch medical jargon/ skirting around difficult issues

After a really tough week (and attempting to hide symptoms for several weeks) finally feeling half human again today. Also chuffed, scared but excited in equal measures to have been appointed as pathology clinical director today for the health board. Exciting times once fit again
I was seen out this afternoon and someone who I thought was a very supportive friend from my village asked why I am off on sick leave when I can be out and about..... Wow. Will be housebound until the 5th June when I return!

A really great feeling in work today. A busy ward round with close to 25 patients but not a single active/ recovering covid case amongst them. Lots of sick liver patients, GI bleeds and general Gastro cases but it felt like the first bit of mormality in almost 12 months

What the hell is wrong with the NHS in making it so difficult and frustrating to take up a new job, even within the same health board. ID checks (needing 4 pieces of ID), CRB check (the person works for you every day now) and occ health forms (including vaccines).

The stress and extra hours of work at night to catch up on 200+ emails, a folder full of a weeks worth of admin backlog and multiple queries etc whilst doing normal clinical duties.and feeling swamped 2 days in makes annual leave almost totally pointless taking.
A sombre evening seeing 3 of my pals 2 metres away on 4 separate tables in the village pub. Said goodbye for probably a month plus. Long winter ahead here we come. All of them said we don't get the rules but we know they may help you and your colleagues so we will follow them
I get there is concern, apprehension, even distrust and worry about some PA colleagues. But... can the medical profession try and maintain some sort of balance. They aren't all charlatans and without our team PA we would fall over. They don't claim to be anything other than a PA!

Tomorrow it's work as normal but supporting nursing colleagues (including my wife) who have taken the horrendously difficult decision to take industrial action. We will ensure essential services run as planned. Their working conditions. The vocation argument has dried up too
Remarkable to think in over a 100 years nurses have never gone on strike across the UK. That could change in the weeks to come. Some trying to blackmail them asking "Is this responsible as we head into Winter". Why not ask politicians if it's responsible to drive them to this?
Today our hospital said goodbye to a brilliant doctor, friend and human being. Some attended in person, some together in our lecture theatre, others online between cases and clinic. Some couldn't but reflected in silence. Every one of us will miss you Brad. Cysgwch mewn hedd x
Today I had 35 patients under my care. Our team consisted of 1 Physician Associate and one trust doctor. We managed to discharge 8 patients, reviewed 3 patients on our ambulatory unit, scope 4 UGI bleeds and a food bolus and left almost on time with a smile. One hell of a team ☺️

Scary how quickly things have changed in hospital in the space of a week re Covid. Yes absolute numbers remain relatively small but to know we now have two covid wards and a lack of capacity for other admissions fills me with dread for the winter ahead. This is going to be tough!
The end of another tough week but made so much better this afternoon by being thanked for breaking bad news in a "good way". The patient and their family had seen a clip I had filmed. This wasn't why I did it but being told that was amazing. We both cried and had s virtual hug 😊

Returned from a fantastic 2 weeks off but this evening already on edge. A packed clinical week, 489 emails to get through, over a 150 results to review and write on as well as other admin, queries etc. It's almost impossible to enjoy A/leave in the NHS knowing you return to this
Returning to work today very disheartening. Seeing managerial and bed management team broken is not nice. Test, test, test is all very well and good but without national strategy to manage inevitable staff absences/asymptomatic cohort requirements and assoc chaos we're in trouble

On 1 of the hottest days of the year satisfying performing 5 ERCP's and duondenal dilatation today. All wearing lead apron/full PPE3 . It's a superb diet and am currently 3 stone lighter and a foot shorter... Can't think how bad it must be to be "forced" to wear a mask in a shop

A very tired team this evening who had an overwhelming response on day 2 of our liver health tour in Merthyr Tydfil. 166 fibroscans performed and each an opportunities to offer liver health advice. 30 identified with abnormal fibroscans booked into F/U clinic!

Today's announcement by WG saying patients can/should be discharged without the care packages they require reflects the dire state we are in. Worryingly it also shows a lack of understanding as to what this means for clinicians when complaints/litigation/GMC referrals come in 🤔
Start of a busy few days in Santiago, Spain learning all things EUS and Pancreatic insufficiency. Not the best of starts with an alarm failure at 4am and sprint through Gatwick followed by a bumpy blind landing in fog. Hopefully a calmer 3 days ahead!

Both my parents were teachers for over 30 years. I have been a governor at a superb school and seen the stress inspections cause. The system is not fit for purpose at present and hasn't been for year... It reeks of those who can teach, those who can't become an inspector
