/10 it wasnât easy, lots of changes to clinical activity but no doubt senior decision makers did make a difference However it took time, willingness, compromise & stepping into different zones Its worth a reflection (or few) on how they work & what we can do to change old habits

/9 senior decision making is clearly better. This is better in person & makes it more seamless for the pt. Being more present & involved meant a lot less faff,a lot less phone calls & actual makes for a pleasant pt experience. Be nice to see this happen more often

/8 getting a second opinion is so easy isnât it when a boss calls a boss. The contrast experience when juniors make the same call is mind boggling. More bosses should make the calls clearlyâŚ.save your junior getting the short end of it. Respect works two ways btw

/7 talking to some of the seniors who also did ward cover, interesting to hear how they were able to rationalise what was needed for their patientâŚwhy donât we do that every day? Do it in the day that leaving an uncertain decision in the eveningâŚ

/6 continuing with Ix; do we ever stop to think âwhyâ? I donât think we do. Maybe working with surgeons has made my thought process a bit more streamlined but I will always ask why a blood test is done out of hours if itâs not indicated. Hope more senior docs âthemselves more

/5 amount of work we create for our juniorsâŚtests that donât need to happen at that time & how itâs ok to not ask for a test. My ward cover shift were slightly eye opening for that.I can ask â& make decisions, but can defer with experience. We need to make that easier for JDs

/4 stepping up meant going into slightly unfamiliar territory but not having any ego about asking questions. That was me in sorting a DC from ED or prescribing certain meds on EPRâŚthis was the same for many of us. That included the choice to do nights đđ˝đđ˝ @AcutemedSarbc

/3 lots of discussions about consultants stepping up down; those willing to help were plenty. Those with trepidation was real too it least it shows weâre human..& yes, admit to the fact we donât do TTOs/request tests. Having a @EdwardFogden on side got us sorted & lots engaged â

/2 how to approach your juniors with regards to the situation; approach with kindness, donât corner them! Having heard of experiences of other trainees elsewhereâŚI hoped we managed it well, having a @Mikeblaber as your sidekick helped ++
