It wasn't a miracle. He had a sudden cardiac arrest with a shockable rhythm, received enough oxygen to his brain to preserve it through CPR, and was shocked back into rhythm.
The supervisor of a poorly behaving physician said to me today “we should be mindful that this is a stressful time for him b/c his wife is almost due with his first child”
I pointed out I worked clinically through 74 weeks of combined pregnancy without a professional complaint.

. 60% of my census are kids admitted with COVID or MIS-C. All unvaccinated. Several intubated, some previously healthy. Parents crying. Spent 3 hrs intubating and placing lines last night. I’m so so tired. And we havnt even peaked yet in Oregon.
Just visited my sons class to talk about being a doctor. Their favorite part? Laying in a circle with their eyes closed pretending to be asleep until the pager in the middle of the circle goes off.
Because when we use miracle language to describe predictable outcomes as a result of tested and reliable interventions, we give the person in completely different medical circumstances hope that they'll get the same miracle, because medical miracles are “happening all the time”
because their pastor describes every medical recovery as a miracle.
Lots of believers suffer prolonged excruciating disease and existential crisis waiting for that miracle because of this misuse of miracle language.
Also, it makes the medical staff's job a lot harder.

at her pediatrician today! People worry about the number of shots, but did you know we're exposed to thousands of antigens every day? Vaccines are a tiny drop in a big bucket with the added bonus of protecting our littles against serious illness.
Daycare closed for COVID. My out of office message: I will be unable to attend meetings/answer emails as the US has systematically failed to protect young children and working parents. I will respond when my child is vaccine eligible, which may not be until 2023.

A miracle would be if he had gone 20 minutes without intervention, long enough for his brain to die, and then spontaneously revive fully intact. Why is this language so important?

Grateful for the kind woman sitting in front of me on Alaska 1116 who, when I tried to stop my 18 mo from kicking her chair and opening and closing the tray table, said “I sincerely don’t mind. I consider it a free massage. I’ve been there. Happy thanksgiving”
I foolishly thought the worst part of parenting was the newborn phase. But now I know it’s actually when your child drops their nap and becomes the intolerable spawn of satan everyday between 4 and 7pm.
On a FB group for physicians, a woman asked about strategy for taking the most considerate maternity leave. A male physician responded with the comment below. I had to check my calendar to make sure I was still in the year 2023. There’s no excuse for this mentality.

8/Zoey’s parents are fortunate to have connections and flexibility to take time away from work to fight these road blocks. Imagine being too sick to advocate for yourself, or non english speaking, or unable to afford taking hours off work to wait on the phone.

One of my colleagues knows their tendency to act like a jerk when stressed. We have a code word we say when we need them to readjust quickly. No big conversation in the moment. Just say “bacon” and the magic word is enough to make them self aware enough to right the wagon.

5/ 5.Zoey’s doctor was able to schedule a “peer to peer” and arrived on the call to discover the “peer” was a PharmD with no oncology expertise. This was the same PharmD who had denied the prior authorization the last two times. The PharmD again, denied the appeal.

then contacts an investigative journalist who notified BC of her involvement and requested appeal documents. Within hours of being contacted, BC approved Lorlatinib with a $5k/month copay and a letter that Zoey will have to undergo review again in 90 days.
2/Zoey requires surgery but her tumor is too large to safely be removed. Biopsy shows she needs a targeted drug, Lorlatinib, to shrink her tumor first. Tumor boards at two separate nationally recognized children’s hospitals agreed. Now watch how many times it was denied by BC:
This is my hospital system on lockdown with an active shooter. My friends & coworkers are there, texting w/ me afraid and devastated about the fatalities that are rumored but not yet confirmed. Our calling is to help people. We shouldn’t be afraid to come to work. ENOUGH.

On Monday morning after rounds, I felt like I got stabbed in the pelvis with a samurai sword. I was quickly diagnosed with a ruptured ectopic pregnancy and rushed to surgery. They drained 750cc of blood from my pelvis and stopped the hemorrhage. Here are my reflections:
3/ 1. After 2 weeks of waiting,

2. Eunice filed expedited 72 hr appeal. BC denied, stating request “not urgent” and sent to regular 14d appeal. After days calling to escalate, BC agreed to expedited appeal
4/ 3.

then sent appeal to third party company for evaluation, MCMC. They recommend denial, stating this drug is “investigational, not standard of care, and is not medically necessary” and kicked it back to BC
4. BC took the recommendation and denied the appeal
I can think of no other field besides medicine where you are expected to provide a service despite being verbally abused for 12 hrs by the family receiving that service…. And then be expected to come back again in another 12.

The hardest part of being a

is childcare. Hands down. I just received 2 hr notice to pick up my baby because her teacher is sick & they can’t find a sub. Closing her classroom all week. She’s had 8 days of daycare in the last 3 weeks. Unsustainable.
A consulting doc asked if we could talk privately in my office. When she got there, she said she just needed a safe place to cry for a minute because she was just. so. tired.

I called a nephrology consult and she came to the

with a Tupperware filled with her own manual BP cuffs of all sizes and it’s literally the highlight of my day.
The end of a known pregnancy is a surprise to no one. A responsible practice should have contingency plans starting at 36w. If someone has to go out earlier, be a respectable human and cover for them. Irked about missing time with family? Reflect on how that family came to be.
When I was a little girl, I dreamed that one day…if I worked hard enough… I could become a doctor… so I could spend 2 hrs in the middle of the night arguing with parents who would like to treat their child’s blood pH of 6.9 exclusively with naturopathic remedies.

I must acknowledge the inherit privilege in my good outcome. As a white attending, no one dismissed my pain. The death rate of ruptured ectopic pregnancies is 7x higher in Black women. Black pain is often dismissed. We must do better. ALL women deserve to have a good outcome.
Finally met my new PCP, she told me she’s leaving too in the coming months.
Told me her RVU minimum set by her employer is so high that, paradoxically, if she takes her PTO then she wont meet her astronomical RVU goal and has to pay back her employer at the end of the year.
I’m searching for a PCP for the 4th time in 3 years. All have left primary care. I asked my doc where she is going and she said “I don’t know yet, but I know I can’t do this anymore”.
Soon primary care will only be accessible to those who can afford DPC/concierge.
