These intercepts are generally aircraft operating VFR. Pilots operating VFR are often not in contact with ATC. They’re supposed to monitor Guard (121.500) but often do not. Generally a headbutt maneuver is deployed when the aircraft does not answer calls on Guard.
Unfortunately, a headbutt is not uncommon. Pilots are commonly on the wrong frequency and the headbutt is one of the first steps in an intercept procedure to get their attention in the "post intercept phase" (AIM 5-6-13 Intercept Procedures)
