CTO cap, body, cap crossed. IVUS showing intraplaque course. Ballooning then DES deployed with final overlapping ostial stent placed using RTIG. Good ostial position with no geographical miss. Uncomplicated procedure < 90 mins, 100mls contrast, msa 12mm2, same day discharge.

We went for ADAPT 3D wiring. Single access radial 7.5F AL1 Asahi SH guide. Turnpike MC, Gladius EX14, Gaia 3rd next. I'm not so good at trigonometry anymore, but such tiny movements of the wire tip based on the clock-face, with attention to the far field view & orthogonal planes.

Proximal RCA CTO with ambiguous cap on IVUS, lots of bridging collateraks and only visible retrograde collateral from apical LAD. Options: side-base with knuckle and ADR vs retrograde through apical LAD collateral vs AWE.

APH in late T3 which precipitated c-section. Mum and baby well. Switched back to warfarin post c-s. TTE late T3 below. Nice example of providing gold standard of care to a patient despite pregnancy status. Case published in Obstetric Medicine

What we did. MDT discussion, all risks discussed. Patient opted for mechanical AVR. Underwent successful mechanical AVR / Bentalls at 14 weeks gestation. Onyx valve to allow INR 1.5 - 2.0. BD LMWH 1mg/kg through pregnancy with Xa level monitoring.
