Crash intubation in A&E the other night. Urgent ++, so we observe a shortened checklist. 20 seconds later we’re ready to RSI, having identified suction not switched on. If you think you don’t have time to do the checklist, the more you need to do it. #patientsafety

This won’t surprise anyone who sees emergencies in cardiac lab. - Unfamiliar, unmarked kit & storage.- Unclear objectives between cardio/anaesthetic team.- Variable (!) comms.- Horrible ergonomics . Could it be improved with better standardisation, joint sim & debriefing?.

Out now!.More NAP7.Cardiology patients receiving anaesthesia.-often an unplanned event.-logistically high risk (remote & unfamiliar environment?).-medically high (>1% of anaesthesia activity vs almost 10% of cardiac arrests).-poor outcomes . (Contrasts very much with cardiac.

@EM_RESUS This is the reason London telephone boxes (the red ones with the windows) were banned . They were death traps because the elderly fainted inside from the heat, and then were prevented from falling to the ground because of the 4 walls. Every morning there were a few elderly dead.

Spent a fascinating day at an aircrew CRM course, welcomed by @mike_sooty and @draken_europe 🙏. Lazy parallels between healthcare & aviation are common - but emulating nuances of personal reflection, openness in error & debriefing (good & bad!!) in NHS are v achievable. ✈️.

Do other ppl now find Twitter really difficult to engage with, particularly on #medtwitter stuff? . I’m rarely presented with stuff I’m interested in or ppl I used to closely follow. Not sure if it’s compounded by my own disengagement or Elon’s failed algorithm?.
