Tough M1 occlusion from Trousseau's synd. 3 passes-3 different strategies: anterior-division combined technique → posterior-div combo → aspiration only. With cancer-related clots, I dont repeat the same maneuver. Each pass teaches us clot's behavior. Each pass needs a new plan.

Numerous people have asked so I figure I’ll mention here: - totally ok to do on patients awake. About 20cc lido — use longer needle to number deep depending on habitus - can access SSV in upper calf the same way. May be partially limited on sheath size, but its an option for

Retrograde transvenous obliteration - humbled by this one due to complex outflow. Planned on a quick outflow plug and slurry, only to have high flow phrenic escape. Dual micro access through phrenic allowed occlusion and retrograde fill to SV.

Most people don’t realize ….
Technical note about the ’14 Fr’ chest tubes now popular in Trauma: The ’14’ may be a bit of a euphemism, as the inner diameter of the tube is closer to 9 Fr, and the diameter of the narrowest point (the stopcock) is even a bit smaller than that. 🤔🧐

15 yr occluded/embedded Simon-Nitinol looked like it was jumping to click its heels - the patient however was not. Complex IVCF retrieval and vein reconstruction restored physiologic flow and immediately improved symptoms. #chogazi @drochohan @mistermudarads
