Conclusions: Splice-altering variants affecting non-canonical positions in TTN explain 2% of DCM and offer a 10-20% increase in the diagnostic power of TTN sequencing in this disease. Trends in data suggest rules that may improve detection of similar variants in other genes.

Key findings: 1⃣ Splice altering VUS in TTN were present in 2% of DCM cases (0.2% of controls) 2⃣ 20% of VUS in TTN splice regions affect splicing and are unrecognized pathogenic variants 3⃣ +5 position in donor site, -3 position in acceptor site most commonly affected

We studied rare variants of unknown significance (VUS) located in the splice regions of highly expressed TTN exons from: ➡️203 DCM cases ➡️3329 normal subjects, and ➡️clinical variant databases The effects of these variants on splicing were assessed using a minigene assay.

TTN truncating variants (TTNtv) are the leading genetic cause of idiopathic DCM. But patients undergoing genetic 🧬testing are found to have synonymous, missense, or intronic variants in splice regions of the TTN . How many of these affect splicing and are unrecognized TTNtv?
