1/3 Rising documentation burden has led to strategies like the digital scribe. The DS uses AI to record and organize info into an office note format. Clinicians would review + edit before signing. Our pilot assesses the DS in cancer care.

5/5 The effectiveness of EHR nudges for implementation vs. de-implementation of care practices represents a future line of inquiry. @avaneeshkunta @YoungRock_H @KarimHannaMD @RandaPerkinsMD @TurnerKea.

4/5 Another interesting finding was that most of the evidence involves EHR nudges to increase uptake of specific behaviors. Attempts to use EHR nudges to de-implement specific care practices were more scarce.

3/5 The evidence was less consistent for patient safety, effectiveness, and patient adherence measures. Lastly, most studies found null associations for efficiency, patient-reported outcomes, and actual clinical outcomes.

2/5 Overall, we found that there were many studies showing an association between EHR nudges and documentation behaviors. Of the few studies on patient-centeredness measures, all showed associations with improved outcomes.

1/5 EHR nudges have been increasingly studied in primary care and there is a need to summarize how nudges have been associated with different dimensions of healthcare quality (e.g., patient safety, efficiency of care). Our recent review aims to do so.

5/4 It also seems unclear if documentation expectations, workflows, and how to document with nurse-specific tools (e.g., flowsheets, care plans) are routinely covered in EHR training programs. Special thanks to @HwayoungCho and @kenrickcato for helping getting this paper out!.

4/4 Training was often in connection with onboarding or implementation. One interesting question raised is how should RNs best receive EHR re-training or updated workflows if they don't have dedicated resources or time carve-outs from patient care to receive this info?.

3/4 Other interesting patterns included lack of clarity in how current nursing programs' EHR preparation influences design of hospitals' EHR training programs and lack of studies on how RNs should receive periodic EHR retraining or updates when the EHR changes.

2/4 Using the CFIR, we mapped factors of EHR training programs targeting inpatient RNs. We found several patterns in characteristics that may contribute to effectiveness, such as personalizing training, breaking up training over several days, and using trainers who are/were RNs.

1/4 EHR training programs are effective at improving self-efficacy among clinical staff, including RNs. Now the question is what program design or implementation characteristics contribute to their effectiveness. Our recent review examines this.

2/2 This suggests that telehealth may partly improve the access to care gap for people with impairments. Additional research is needed on the telehealth patient experience among this patient group.

1/2 Telehealth use has expanded but it use may differ based on type of patient, such as those with vision impairments 👓. Using 2020-2022 NHIS data, we found a stepwise increase in odds of telehealth use as severity of impairments increased.
