The irony here is that this guy is obese based on BMI, his BMI is 31.4. One of the issues around weight and people maybe not recognising the health risks.

So I’m not getting a vaccine next week - was feeling weird about why I’d been selected ahead of others so rang GP to check. Turns out they had my height as 6.2cm rather than 6 ft 2, giving me a BMI of 28,000 😂

Never understood why @NICEComms recommended continuing until delivery, when the main benefit is a possible reduction in early delivery from pre-eclampsia. I understand they did this in the studies, but just stop it at 36 weeks!

“When I qualified, I wish somebody had told me, ‘you will make mistakes, & they will cause harm -here’s what you need to do about it, for both yourself and those you care for’ - but they didn’t.” Dr Bill Kirkup talks about the need for transparency & honesty when things go wrong.

Witnessed the most amazing thing on the train to Edinburgh yesterday. A guy boarded in Wigan & sat opposite me. He went to sleep for an hour. When he woke up he bought a sandwich, ate it & went back to sleep. (This isn’t a maths test, you don’t need to know the distance/speed).

For me as a consultant obstetrician this is my biggest headache, not being able to deliver at risk premature babies when we think they need to be, and spending hours arranging transfers hundreds of miles away, almost exclusively due to nursing shortages IMO https://t.co/MZMIIStgdn

I meant what I said: It’s not good enough & I am sorry. I love the NHS & I always want it to be the best. I want to thank the NHS team in Leeds, responding to unprecedented demand. We will treble the size of the unit next year & then rebuild the LGI.

And what do you do after 40 weeks? It looks to me like EFW and HC/AC/FL charts end at around 40 weeks, but it isn't uncommon to scan at 41 or 42 weeks in patients who are keen on conservative management due to wishing to avoid IOL?

- What do you do if the sonographer feels the HC they get is inaccurate and doesn't wish to use it to calculate the EFW (the IG-21 formulae is a function of AC and HC and doesn't include FL)?Do you base management on AC centile and not use EFW?Are you using Hadlock AC-FL instead?

Couple of practical questions for any units who are using Intergrowth-21, all comments welcome! - I assume everyone uses the IG-21 EFW formulae, any concerns some small studies suggest a larger mean error compared to birthweight than Hadlock? (1/..) @jimgthornton @tr_everett

Shocked to discover otherwise respectable maternity units using #customised fetal size charts? You should be. Time for a series of blog posts explaining why. Here's the first. https://t.co/A61TLV2aKm 1/16 More tomorrow.
